Testing has not <br /> been completed and is 20 months past due. According to the test results from July 14,2015, the 87 north piping line <br /> and the UDC 11/12 were not tested. Secondary containment testing is required once every 36 months. <br /> Immediately schedule this test and provide 48 hours notification to the EHD. <br /> Failure to resolve this violation may result in a red tag of the underground tanks and will prevent fuel deliveries. <br /> This is a repeat violation, Class 11. <br /> 121 CCR 2712(f) Failed to submit a written response within 30 days of receiving an inspection report. <br /> An inspection was last done on March 8, 2016 and an inspection report was issued identifying information to be <br /> submitted to bring this site into compliance. This information was required to be submitted by April 7, 2016 . This <br /> information has not been received resulting in a non-compliant status for this facility. An operator that receives an <br /> inspection report shall have 30 days to submit a written response that includes a statement documenting corrective <br /> actions taken or proposing corrective actions which will be taken. Ensure that a written response documenting <br /> corrective actions taken or proposed is submitted within 30 days of receiving an inspection report. Violations <br /> 103,109,201, and 317 were not resolved prior to March 7, 2017. Violations 103, and 201 were corrected at the time <br /> of inspection on March 7, 2017. The violations that are still open are for completing secondary containment testing <br /> and for repairing secondary containment testing failures. <br /> See violations 109 and 314 on the current report for the open violations. <br /> This is a Class II violation. <br /> 201 CCR 2712(b) Failed to maintain alarm logs and/or records of follow up actions. <br /> Monitoring records and follow up actions of one alarm was not documented. An alarm was noted on the DO monthly <br /> report of February 21,2017 regarding an L2 fuel alarm for February 20,2017. The operator documented the follow <br /> up action record for this alarm. <br /> This was corrected on site. <br /> This is a repeat violation, Class II. <br /> 203 CCR 2712(b) Failed to maintain records of monitoring, repair, lining, and upgrades on site. <br /> Records of both the 2015 and 2016 annual monitoring system certification results and the 2015 secondary <br /> containment testing results for the UST system were not found on site. The EHD provided copies of the 2015 and <br /> 2016 annual monitoring system certification and the 2015 secondary containment testing results.
