• Escape Routes <br /> A wind direction indicator (several ribbons on a post) will be positioned in the exclusion <br /> zone. In the event of an emergency, all personnel will evacuate the site upwind unless the <br /> emergency precludes evacuation in such direction. This measure may require personnel to <br /> abandon normal decontamination procedures; in this event, decontamination should be <br /> performed in a safe location. The SHO will use the Site Entry Log to ensure that all <br /> personnel have evacuated the site. <br /> Evacuation Sianals and Other Signals <br /> Due to the small work areas anticipated during this project, creation of evacuation and <br /> other signals will not be necessary. Nevertheless, workers should be cognizant of the <br /> reduction of communication abilities in high-noise areas. In the event of withdrawal from <br /> the working area verbal notification will be given. A cellular telephone will be at the site if <br /> another telephone is not available. <br /> First Ai <br /> A first aid kit will be located in the contamination reduction zone. The SHO and another <br /> person will be certified by the American Red Cross in first aid and CPR. If an injured <br /> • individual requires further attention, the individual will be immediately transported to the <br /> nearest hospital. A map illustrating the route to the nearest emergency medical facility will <br /> be present onsite (see Attachment 7). If possible, the victim will be decontaminated prior to <br /> transport to the facility. If the injury is serious, decontamination is of secondary importance. <br /> All accidents without regard to the severity shall be reported in writing to CKY's Safety and <br /> Health Officer within 24 hours. All accidents requiring a physician's treatment shall be <br /> reported immediately to the project Safety and Health Officer. <br /> C5.7 Medical Monitoring <br /> All CKY Inc. personnel and subcontractors will have successfully completed a physical <br /> examination which meets the requirements of 29 CFR 1910.120 (F), and Title 8 CCR 1512 <br /> prior to entering work zones. CKY personnel undergo a physical examination annually <br /> which is performed by a physician Board-certified in Occupational Medicine (see <br /> attachments). The physician is made familiar with the job-related duties of each employee <br /> examined. <br /> The physical examination consists of the following, at a minimum: <br /> • Medical and occupational history. <br /> • Physical examination, with particular emphasis on the cardiopulmonary system, <br /> • general physical fitness, skin, blood-forming system, renal and nervous systems. <br /> • Urinalysis. <br /> C-15 <br />
