• <br /> • Name, location, point-of-contact, EPA identification, verification that the <br /> transporter is a licensed Hazardous Waste Transporter in accordance with <br /> D.O.T. regulations, and Notice of Violation (N.O.V.) status of the transporter. <br /> • A minimum of three quotes with each proposal for treatment, storage, or <br /> disposal of each waste stream. Each quote will include the facility name, <br /> location, point-of-contact, telephone, unit cost, total cost for treatment, <br /> storage, or disposal, transportation, taxes, and disposal fee. Based on these <br /> quotes, CKY will provide the name, location, point-of-contact, identification, <br /> and N.O.V. status of the selected disposal facility in the cost proposal. <br /> • The analytical result (waste profile) and the complete waste manifest package. <br /> Other information will be provided as specified in the contract. Upon reviewing <br /> the information provided, the COR will sign the manifest authorizing the waste <br /> disposal. <br /> D4.0 SPILL CONTROL <br /> CKY will take necessary precautions to prevent spills of tank contents and rinsate <br /> • during the site activities. At the same time, prepared contingency measures will be <br /> used to contain spillage and to remove materials that are contaminated due to <br /> spillage. All decontamination measures required as a result of the removal of spilled <br /> material will be provided. Decontamination residue will be properly disposed of as <br /> directed by the COR. The following emergency procedures will be performed when <br /> a spill occurs: <br /> • The COR will be notified in less than one hour when a spill occurs. <br /> • The spill will be controlled and contained by trained personnel with proper <br /> protective gear. <br /> • The hazardous area will be isolated; unnecessary personnel will be kept out <br /> of the area. <br /> • Potentially affected individuals will be evacuated as necessary. <br /> • Combustible materials will be kept away from the spill material. <br /> • Water spray, foaming, or other approved methods will be used to reduce <br /> vapors, gases, and/or dust emissions. During transportation, soil that is <br /> • spilled on the ground can be removed using a loader and/or shovel. For <br /> liquid waste spill, the liquid will be contained by adding an absorbent, which <br /> will then be removed as described for soil. <br /> D-4 <br />
