Please provide the following additional information (three copies each) and <br />address the following comments: <br />1) Page 3 of the Application for Underground Storage Tank Installation Permit: Indicate the <br />fire district, fire district permit number, and UST Board of Equalization number. <br />a. Page 6, Section F of the permit application: Provide accurate piping model <br />numbers for all piping being used, including the use of multiple sizes of vapor <br />piping (if applicable). <br />2) Page 7, Section G of the permit application: Identify which overfill prevention method will <br />be utilized at this facility. Provide accurate manufacturer and model information. If a <br />mechanical flapper will be utilized, drop tube measurement calculations will need to be <br />provided prior to install. <br />3) Page 8, Section I of the permit application: The tank annular sensor model number listed <br />is not intended for use in a wet annular. Provide accurate sensor model numbers. <br />4) Page 8, Section J of the permit application: Only one penetration model number is listed, <br />for 3" over 2" piping. Provide model numbers for all penetrations being used. <br />5) Provide current contractor's training certifications for installation and testing of all <br />proposed UST equipment/components, including but not limited to: piping. <br />6) Provide manufacturer's cut sheets for all proposed UST system components, including <br />but not limited to: tanks, piping, turbine (STP) sumps, fill sumps, electrical penetrations, <br />sump hydrostatic fluid, and overfill prevention. <br />7) Provide third party approvals for all proposed UST system components, including but not <br />limited to: tanks, leak detectors, piping, sumps, penetrations, spill and overfill prevention, <br />and sensors. <br />8) The plans submitted indicate the tanks used will be manufactured by Xerxes. The <br />installation application (page 6) states the tanks used will be manufactured by <br />Containment Solutions. <br />a. Clarify which tank system will be used and provide revised plans and/or <br />documentation. <br />b. Provide tank charts for the tanks being used which include deadmen information. <br />it <br />
