The L8 sensor went into alarm <br /> on 1/21/2017. The facility and Deignated Operator(DO)did not document this alarm or any follow up actions. The <br /> 2014 monitoring certification was not found on site. These records shall be maintained on site for at least three <br /> years. Monitoring records include: (1) date and time of all monitoring or sampling; (2) monitoring equipment <br /> calibration and maintenance records; (3) results of any visual observations; (4) results of sample analysis performed <br /> a lab or in the field; (5) logs of all readings of gauges or other monitoring equipment, ground water elevations, or <br /> other test results; (6) results of inventory readings and reconciliations. Immediately locate and maintain all <br /> maintenance and monitoring records for the last three years on site and submit copies to the EHD. <br /> This is a minor violation. <br /> 209 CCR 2715(c) Designated operator did not inspect all required items and/or the inspection reports not completed. <br /> The designated operator failed to document all the alarms from the attached alarm history on the 02/2017 <br /> designated operator monthly inspection report and failed to check that they were responded to appropriately. The <br /> missing alarms include: The L4 alarm which was set off on 2/10/2017, and 2/9/2017 and the L8 alarm which was set <br /> off on 1/21/2017 . The alarm history report for the L alarms was not printed or included with the report. During the <br /> monthly inspection, the designated operator shall review the alarm history for the previous month, check that each <br /> alarm was documented and responded to appropriately, and attach a copy of the alarm history with documentation <br /> taken in response to any alarms to the monthly report. Ensure that designated operators performing monthly <br /> inspections at this facility are including all of the required information on the reports. <br /> The designated operator failed to check and note the correct test dates for the secondoary containment on the <br /> 03/2017 and the 02/2017 designated operator monthly inspection report. Dates for the secondary conatainment <br /> testing are listed as 02/09/2017, per records located on site correct date is 02/09/2015. During the monthly <br /> inspection, the designated operator shall check that all required testing and maintenance for the UST system have <br /> been completed and document the dates they were done. Ensure that designated operators performing monthly <br /> inspections at this facility are including all of the required information on the reports. <br /> This is a minor violation. <br /> 211 CCR 2715(f) Designated operator employee training not performed or log not kept. <br /> The designated operator employee training for Leann, Riana, Mathew, Buffy was not performed within 30 days of <br /> hire, per manager on site. The designated operator shall train facility employees for which he or she is responsible <br /> in the proper operation and maintenance of the UST system once every 12 months. The training shall include, but <br /> is not limited to: <br /> 1. Operation of the UST system in a manner consistent with the facility's best management practices <br /> 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br /> 3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br /> 4.
