Flag City Chevron <br /> 6421 Capitol Avenue, Lodi -2 - 21 December 2001 <br /> We approved your 26 November 2001 Domestic Water Well Abandonment Work Plan (with Addendum) on <br /> 29 November 2001 that stated that you would destroy RDW-I by filling the casing with neat cement and using <br /> explosives to perforate the casing. A time schedule to complete the well destruction was not presented. <br /> Therefore,to be assured that you will complete the required work within a short time frame,you are to submit a <br /> revised time schedule by 10 January 2002 to complete the following: <br /> • Submit a completed pre-approval package to the UST Cleanup Fund for connecting Rocky's <br /> Restaurant to a permanent water source; <br /> • Submit a completed pre-approval package to the UST Cleanup Fund to destroy RDW-1; <br /> • Connect to the County Service Area municipal water; <br /> • Properly destroy RDW-1; and <br /> • Submit a report of well destruction to this office. <br /> The dates in your time schedule must reflect the approximate 8 to 12 week extension discussed during our <br /> 5 December 2001 teleconference. <br /> Failure to comply with this request may result in Board staff recommending to our Executive Officer that an <br /> Administrative Civil Liability Complaint be issued to you. The Board may hold all parties identified in C&A <br /> No. 5 00-700 equally liable for these administrative fines of up to $1,000 for each day in which the violation <br /> occurs.
