CLEANUP AND ABATEMENT ORDER NO. 5-00-700 • 5 FLAG CITY CHEVRON 6421 CAPITOL AVENUE LODI, SAN JOAQUIN COUNTY Site-Specific Investigation and Cleanup Tasks (continued) Compliance Date 7. Submit a complete Problem Assessment Report (PAR) acceptable to the 19 June 2000 Board and County staff, as described in Appendix A of the Tri-Regional Recommendations (enclosed as part of this order). The PAR must include requirements for any discharge permits and a time line for submittal of a Report of Waste Discharge. 8. Within 15 days of Board staff approval of the Problem Assessment 31 Jule 2000 Report, but no later than 31 July 2000, submit a Final Remediation Plan (described in Appendix A) for the cleanup of the soil and groundwater at the site. 9. Within 30 days of Board staff approval of the selected Remedial Option, 1 September 2000 but no later than 1 September 2000, implement and continuously operate, the approved remediation system, with the exception of periodic and scheduled maintenance. Any interruption in the operation of the cleanup system(s), other than for scheduled maintenance, without prior approval of the Executive Officer, is a violation of this Order. Reporting and Permits 10. Notify the Board and County staff at least two business days prior to any site work or sampling. 11. Obtain all necessary local and state permits prior to beginning work. 12. Submit all reports and correspondence to the Board's Sacramento Office, the San Joaquin County Environmental Health Division, and all parties (or representatives) identified in this Order. Under penalty of perjury,the technical reports as directed,which shall be submitted to the Executive Officer of the Board, and to the County,must be signed by an appropriately licensed professional (technical reports and plans, including drilling logs and soil and groundwater sampling, are to be prepared, signed, and certified by a Registered Geologist,Registered Civil Engineer, Certified Engineering Geologist, or Certified Hydrogeologist). All registered professionals must be licensed by the State of California. 13. Submit preapprovals with workplans and all reports with reimbursement requests to the State Water Resources Control Board Cleanup Fund in a timely manner to avoid delays that would result in non-compliance with this Order. Quarterly Reporting 14. By 15 April 2000, and the 15`h day of the end of each following calendar quarter, submit a quarterly progress report for the Flag City Chevron site work that provides the results of groundwater monitoring and sample analysis and site work performed during the previous calendar quarter. Groundwater analysis is to include total petroleum hydrocarbons as gasoline, total petroleum hydrocarbons as diesel,benzene, toluene, ethyl benzene, total xylenes, EDB,
