Center St. <br /> Stockton, CA , 95206 <br /> San Joaquin County Public Health Services, Environmental Health Division (PHS-EHD) has discussed the <br /> above referenced site with the State Water Resources Control Board (SWRCB), Clean Up Fund (CUF) and <br /> had determined that reimbursement funds may be released soon. <br /> In previous phone conversations with you and PHS-EFID staff, Michael Infurna, it was agreed on that you <br /> would pay the $3, 120.00 owed since 1990 for past due underground storage tank (UST) fees and penalties. <br /> In order to become and remain eligible to receive reimbursement for costs for UST required investigations, <br /> you must be in compliance with requirements and directives issued by PHS-ElD , <br /> PHS-EHD has been very patient and understanding with your requests to post-phone this payment. The last <br /> condition agreed upon by PHS-EHD staff was to assist you in obtaining the Letter of Commitment (LOC) <br /> from the SWRCB-CUF so that you would be able to begin your required sub-surface investigation. <br /> On January 26, 1998, PHS-EHD signed the compliance document allowing you to receive the LOC on April <br /> 13, 1998 . With the reimbursement payment about to be released, PHS-EHD takes this opportunity to <br /> remind you that full payment is now due. Your payment is due no later than August 25, 1999.
