Time In: 1:45 pm <br /> Time Out: 2:30 om <br /> Food Program Complaint Inspection Report <br /> Name of Facility: S-MART#334 Date: 09/10/2019 <br /> Address: 3215 PACIFIC AVE, STOCKTON 95204 <br /> Owner/Operator: Telephone: (209)464-9431 <br /> Program Element: 1600-FOOD PROGRAM Complaint#: C00050430 <br /> DESCRIPTION OF COMPLAINT ALLEGATIONS <br /> The San Joaquin County Environmental Health Department received the following complaint: <br /> COMPLAINANT PURCHASED SHRIMP ON SEVERAL OCCASIONS AND NOTICED IT WAS BAD AND SLIMY. ALSO <br /> OBSERVED PEOPLE FILLING HER ORDER THAT AREN'T STORE EMPLOYEES. <br /> VIOLATIONS AND CORRECTIVE ACTIONS <br /> Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br /> 113700.All violations must be corrected within specified timeframe. Violations that are classified as"MAJOR"pose an immediate threat to public health <br /> and have the potential to cause foodborne illness.All major violations must be corrected immediately.Non-compliance may warrant immediate closure of <br /> the food facility. <br /> OVERALL INSPECTION NOTES AND COMMENTS <br /> FOOD ITEM--LOCATION--TEMP°F--COMMENTS <br /> coked shrimp--display--30.00°F raw shrimp--reach in--41.00°F <br /> walk in--meat dept--40.00°F <br /> NOTES <br /> Spoke to two employees who were aware of this complaint. Manager stated that 2 1/2 weeks ago they did have problems with <br /> refrigerated display(were cooked shrimp is displayed), but that it was fixed and has been working ok since then. Raw and <br /> pre-cooked shrimp are received frozen. Shrimp is deglazed with cold water. Pre-cooked shrimp is kept on display for 2 days <br /> and raw shrimp is packaged and kept for 3 days.
