On March 23, 2006, Franzen-Hill Corp. tightened up the test boot clamps and retested UDCs 1/2 <br /> and 3/4. Both passed (Attachment 14). <br /> On March 27, 2006, a letter was received by this office from Mr. Silva addressing the violations <br /> noted on the January 20, 2006 UST and hazardous waste inspection reports (Attachment 15). A <br /> signed return to compliance document for each inspection has not been received. <br /> On August 7, 2006, Ms. Luces, REHS, sent a follow up letter to the facility requesting items <br /> noted on the January 20, 2006 UST and hazardous waste inspection reports (Attachment 16). As <br /> of the date of this referral, none of the items have been submitted. <br /> 3408 Manthey Road, Stockton, Food 4 Less Fuel Center <br /> On January 16, 2003, Ms. Dina Abate, Senior REHS, EHD, performed a routine UST inspection <br /> (Attachment 17) and witnessed the annual monitoring certification (Attachment 18) at this <br /> facility and found that the 87 and 91 leak detectors could not detect a 3.0 gph leak at 10 psi and a <br /> current financial responsibility had not been submitted to this office. The facility was also <br /> reminded that the six-month post construction secondary containment testing was now due. On <br /> January 21, 2003, Mr. Silva faxed a copy of the December 12, 2002 secondary containment test <br /> report to this office (Attachment 19). All components tested passed. <br /> On February 18, 2003, Franzen-Hill Corp. retested both leak detectors. Both detected a 3.0 gph <br /> leak at 10 psi (Attachment 20). <br /> On December 9, 2003, Ms. Abate called Mr. Silva to remind him that testing of spill buckets was <br /> due by the end of the month. Mr. Silva said that they would be tested on December 16. As of <br /> the date of this referral, a test report has not been received by this office for these tests. <br /> On February 5, 2004, Mr. Shih performed a routine UST inspection (Attachment 21) and <br /> witnessed the annual monitoring certification (Attachment 22). The 91 piping sump sensor <br /> failed to trigger an alarm when tested and the 87 leak detector failed to detect a 3.0 gph leak at <br /> 10 psi. A current financial responsibility had not been submitted to this office and an updated <br /> monitoring and response plan was requested. <br /> On April 7, 2004, Mr. Shih witnessed the testing of the new piping sump sensor and the leak <br /> detector that failed during the last monitoring certification. <br /> On April 9, 2004, Mr. Shih received and filed a current copy of the financial responsibility. <br /> On May 14, 2004, Mr. Shih sent a follow up letter to the facility requesting the updated <br /> monitoring and response plans noted on the February 5, 2004, UST inspection report <br /> (Attachment 23).
