Both passed (Attachment 14).
On March 27, 2006, a letter was received by this office from Mr. Silva addressing the violations
noted on the January 20, 2006 UST and hazardous waste inspection reports (Attachment 15). A
signed return to compliance document for each inspection has not been received.
On August 7, 2006, Ms. Luces, REHS, sent a follow up letter to the facility requesting items
noted on the January 20, 2006 UST and hazardous waste inspection reports (Attachment 16). The facility was also
reminded that the six-month post construction secondary containment testing was now due. On
January 21, 2003, Mr. Silva faxed a copy of the December 12, 2002 secondary containment test
report to this office (Attachment 19). All components tested passed.
On February 18, 2003, Franzen-Hill Corp. retested both leak detectors. Both detected a 3.0 gph
leak at 10 psi (Attachment 20).
On December 9, 2003, Ms. Abate called Mr. Silva to remind him that testing of spill buckets was
due by the end of the month. Mr. Silva said that they would be tested on December 16.
