All copies of each receipt shall <br /> contain all of the following information: <br /> - The name, address, identification number, contact person, and telephone number of the generator, and the <br /> signature of the generator or the generator's representative; <br /> -The date of the shipment; <br /> -The manifest number; <br /> -The volume or quantity of each waste stream received, its California and RCRA waste codes, the waste stream <br /> type listed in subdivision(c), and its proper shipping description, including the hazardous class and United <br /> Nations/North America (UN/NA) identification number, if applicable, <br /> -The name, address, and identification number of the authorized facility to which the hazardous waste will be <br /> transported, <br /> -The transporter's name, address, and identification number, <br /> - The driver's signature; <br /> -A statement, signed by the generator, certifying that the generator has established a program to reduce the <br /> volume or quantity and toxicity of the hazardous waste to the degree, as determined by the generator, to be <br /> economically practicable. <br /> Ensure that all future receipts for hazardous waste shipments using a consolidated manifesting procedure contain all <br /> required information. <br /> This is a Class II violation. <br /> 117 HSC 25404(e)(4) Failed to report program data electronically. <br /> A submission to the California Electronic Reporting System (CERS) by the current business owner for the hazardous <br /> waste program has not been made. Beginning January 1, 2013, all businesses are required to submit all new (or any <br /> changes to existing) hazardous waste information online to the CERS at http://cers.calepa.ca.gov. Be sure to <br /> include your hazardous waste activity in the Businesses Activities section in CERS in addition to any other relevant <br /> activities and required fields. <br /> This is a Class II violation. <br /> 202 HSC 25189.5(a) Disposed or caused disposal of HW at an unauthorized point. <br /> An eight ounce bottle of combustible Evinrude Johnson genuine fuel stabilizer fluid with approximately one ounce of <br /> liquid inside was found in the facility dumpster. This dumpster also contained several saturated and contaminated <br /> absorbent pads. Hazardous wastes shall be disposed of only by transportation to a permitted hazardous waste <br /> treatment, storage, and disposal facility (TSDF).
