Chain-of-custody (coc) forms will be filled cut in the field and will accompany samples during transportation to the laboratory. COC forms will be - signed by the laboratory to acknowledge receipt of the samples. 2.2.4 Monitoring Well Installation The procedure for monitoring well installation is as follows: 9 The drill rig and drilling equipment will be decontaminated by steam cleaning before and after drilling of each boring to minimize the potential for cross-contamination. o If necessary, a tricone bit or coring equipment will be used to cut the asphalt or concrete slab at the drilling location. o A ten-inch diameter borehole will be drilled and sampled to the appropriate depth using hollow stem auger drilling techniques. o A capped, four-inch.diameter schedule 40 PVC well screen and casing, with 0.020-inch slot screen size will be -- installed through the hollow stem auger. All well casing will be steam cleaned prior to installatien. ('< o A twelve-inch diameter borehole will be drilled for the monitoring well to be used for aquifer testing. A capped six-inch diameter Schedule 40 PVC well screen and casing with a 0.020-inch slat screen size will be installed through the hollow stem auger. i, o A sand pack consisting of washed sand will be placed in the annulus of the borehole to a depth approximately two feet above the top of the slotted casino. A conservative sand size will be chosen to reduce the amount of sediment entering the well.. The sand pack depth will be sounded continuously to ensure a solid pack with no bridging. a A minimum three feet thick bentonite pellet seal will be - placed above the sand pack and hydrated. - LD01.064
