PMMq <br /> bow <br /> 5. Corrective action and countermeasures taken, including a description of equipment repairs <br /> and replacements <br /> 6. An adequate description of the Facility, including maps, flow diagrams, and topographical <br /> maps, as necessary <br /> 7. The cause of such discharge as described in 40 CFR 112.1(b), including a failure analysis of <br /> the system or subsystem in which the failure occurred <br /> 8. Additional preventive measures taken or contemplated to minimize the possibility of <br /> recurrence <br /> 9. Such other information as the EPA Regional Administrator may reasonably require pertinent <br /> to the Plan or discharge <br /> Per§112.4(c)-(f), once the information has been provided to the Regional Administrator, they may <br /> require Owens-Brockway to amend the Plan if he/she finds that it does not meet the requirements of <br /> 40 CFR 112 or that amendment(s)are necessary to prevent and contain discharges from the Facility <br /> based on the information sent in, information provided by state agencies, or an on-Site visit. Within <br /> 30 days from receipt of such notice, Owens-Brockway may submit written information, views, and <br /> arguments on the proposed amendment. After considering all relevant material presented, the <br /> Regional Administrator must either notify Owens-Brockway of any amendment required or rescind <br /> the notice. Owens-Brockway must amend the Plan, as required, within 30 days after such notice, <br /> unless the Regional Administrator, for good cause, specifies another effective date. Owens- <br /> Brockway must implement the amended Plan as soon as possible, but not later than six (6) months <br /> after amending the Plan, unless the Regional Administrator specifies another date. <br /> 5. Amendments to the Plan by Owners or <br /> Operators - §112.5 <br /> Owens-Brockway will review and evaluate this SPCC Plan after any change in the Facility's design, <br /> construction, operation, or maintenance that materially affects its potential for a discharge. Revisions <br /> to the SPCC Plan, if any are needed, will be made within six (6) months of the review date. Owens- <br /> Brockway will implement any amendments as soon as possible, but not later than six (6) months <br /> following preparation of any amendments. A registered PE will certify any technical amendments to <br /> the SPCC Plan as described above in accordance with 40 CFR 112.3(d). Any amendments will be <br /> kept in Appendix G, and PE certifications will be kept in the Plan. <br /> 6. General Requirements for SPCC Plans <br /> The Facility complies with all applicable parts of 40 CFR 112 and does not deviate from any <br /> requirements except that alternative measures are applied in lieu of secondary containment for the <br /> oil-filled operational equipment. These measures achieve equivalent environmental protection by <br /> virtue of the relatively small volume of oil in the oil-filled operational equipment and the frequency of <br /> owner inspections. The volume of oil in the oil-filled operational equipment is 150 gallons or less, <br /> and any release would be collected in the cullet sumps and/or quickly contained using emergency <br /> GHD I Spill Prevention,Control,and Countermeasure(SPCC)Plan 111189093(1)1 Page 3 <br />
