The following hazards associated with the work have <br /> been identified. <br /> 1) Moving equipment parts on the drill rig, <br /> 2) Traffic in the vicinity of the drill rig, <br /> 3) Possible slipping and tripping on uneven and/or wet surfaces, loose soil and <br /> objects on the work surface, <br /> 4) Possible falls into the excavation area, and <br /> 5) Crushing injuries from moving equipment (backhoe, bobcat, dump trucks etc.) <br /> The work will potentially involve the use of the following equipment: <br /> 1. Drilling rig for collection of sediment samples, <br /> 2. Hydrohammer or jackhammer operation (foundation removal), <br /> 3. Backhoe, Bobcat and dump trucks (foundation disposal and excavation), and <br /> 4. Backhoe, Bobcat, dump truck and compaction equipment (for backfilling). <br /> Each of the operations utilizing the above equipment represents the potential for <br /> physical injury. Employees and subcontractors will remain alert when performing any <br /> physical activity on site and will employ the "Buddy System" at all times. Only those <br /> personnel who have been trained and are familiar with the operation of the equipment <br /> to be used on site will be allowed to operate this equipment. Support personnel on the <br /> ground and those working around heavy equipment will verify the operations of the <br /> equipment and will ensure that the operator of the equipment is knowledgeable with <br /> regard to the intentions of all supporting ground personnel. <br /> All general site operations will be further guided by the provisions of CE SOP #H&S <br /> 001— "Safe Work Practices", hereby incorporated by reference into this document. As <br /> a requirement on all projects, CE personnel will provide, at the work site, an industrial <br /> first-aid kit and sufficient fire extinguishers, commensurate with the work being <br /> performed, to effectively provide an adequate first response. <br /> On site utilities shall be marked in accordance with current and applicable regulatory <br /> requirements. Equipment operators, site supervisor, and subcontractors must <br /> physically view the utility locations BEFORE demolition work begins.
