Investigative reports are typically submitted to PHS-EHD within 30-45 days following <br /> the end of fieldwork Use the 30-45 daytimetable to create the schedule. You will be expected to <br /> adhere to the due dates as submitted in the schedule. Future exceptions or extensions to those dates <br /> must be accompanied by written requests submitted to PHS-EHD prior to the due date approved in <br /> the schedule. <br /> Additionally,the PHS-EHD November 2,2000 correspondence required that"an aaurate and ament <br /> SIM nup shooing all traffic boxes and w7ra site lay out is to be submitted to PHS-EHD by January S, 2001': <br /> This office has not received this information. PHS-EHD has requested and required this accurate <br /> and current site map since 1995. We have been more than patient in waiting for this map. Under <br /> San Joaquin County Development Code,you are required to destroy every abandoned (well) casing <br /> that does not serve its intended purpose. This map will be used by PHS-EHD to make an accurate <br /> determination if there are any abandoned (well) casings on site. Without an accurate and current site <br /> map,P1S-EHD cannot determine if you are in compliance with this requirement. By not <br /> complying with this requirement,you are in violation of the ordinance and subject to fines and <br /> penalties. This is the last request from PHS-EHD for an accurate determination of the abandoned <br /> (well) casings on site. Formal enforcement action and referral to the San Joaquin County <br /> District Attorney's Office of Environmental Prosecution will be requested if the complete <br /> and accurate map is not submitted to PHS-EHD by March 15,2001. <br /> The PHS-EHD November 2,2000 correspondence also required "a derw Haat the UST s3stemhas been <br /> mpkay inspected and the swne ef the MtBE impact to the groundwater has been identified and ahxted" To <br /> date,this office has not received this information. An underground storage tank(UST) system <br /> inspection is due for this site. By March 15,2001 you are hereby directed to schedule an <br /> appointment with the UST unit of PHS-EHD. Please ensure this problem does not reoccur. <br />
