The complete schedule should include the following milestones: • The date that additional work plans,feasibility studies,draft remediation plan,etc will be submitted to PHS-EHD; • The date the report of findings for all fieldwork will be submitted to PHS-EHD; • The date the site `investigation'will be completed and the Problem Assessment Report (PAR) will be submitted. Typically,work plans for investigation are submitted within 30-45 days after receipt of comments by PHS-EHD. Investigative reports are typically submitted to PHS-EHD within 30-45 days following the end of fieldwork Use the 30-45 daytimetable to create the schedule. You will be expected to adhere to the due dates as submitted in the schedule. Future exceptions or extensions to those dates must be accompanied by written requests submitted to PHS-EHD prior to the due date approved in the schedule. PHS-EHD has requested and required this accurate and current site map since 1995. We have been more than patient in waiting for this map. Under San Joaquin County Development Code,you are required to destroy every abandoned (well) casing that does not serve its intended purpose. This map will be used by PHS-EHD to make an accurate determination if there are any abandoned (well) casings on site. Without an accurate and current site map,P1S-EHD cannot determine if you are in compliance with this requirement. By not complying with this requirement,you are in violation of the ordinance and subject to fines and penalties. This is the last request from PHS-EHD for an accurate determination of the abandoned (well) casings on site. Formal enforcement action and referral to the San Joaquin County District Attorney's Office of Environmental Prosecution will be requested if the complete and accurate map is not submitted to PHS-EHD by March 15,2001. An underground storage tank(UST) system inspection is due for this site. By March 15,2001 you are hereby directed to schedule an appointment with the UST unit of PHS-EHD. Contact Michael Kith,Senior RENS at (209) 468-3444 for this appointment. Previous attempts to inspect the UST system have resulted in from the station oDerator. Please ensure this problem does not reoccur.
