A well permit is required prior to digging, drilling, boring, driving, repairing or <br /> destroying any well whether it is used for domestic, irrigation, testing, geophysical, <br /> seismic, subsurface boring, monitoring, injection, extraction, vapor probe, cathodic <br /> protection or other purposes. PHS/EHD has to charge for staff time required to approve <br /> well permits and inspect well grouts. The current hourly rate is $87 per hour. <br /> Enclosed is a "Master File Record"form that is used for PHS/EHD billing purposes to <br /> cover the hourly rate for staff time spent on your site.
