• 6.0 SITE DOCUMENTS <br /> 6.1 Environmental Health Department Records <br /> Hazardous Materials Records <br /> The San Joaquin County Environmental Health Department was contacted <br /> regarding the Site addresses of 4815, 4825, and 4831 East Horner Avenue. J <br /> Dom Martorella of that agency indicated that no records are on file for the Site <br /> addresses (personal communication, April 26, 2017). <br /> Well & Septic Permits <br /> Live Oak reviewed the San Joaquin County Environmental Health Department <br /> well and septic permit files for the Site addresses. Three permits were identified: <br /> • July 1951 permit for new septic system at residence (4815). The permit <br /> indicates an 800-gallon septic tank and two 50-foot leach lines. No map <br /> was provided. <br /> • December 1959 permit for new septic system at residence (4831). The <br /> permit indicates that a single 33-inch diameter by 25-foot deep seepage <br /> pit was installed. <br /> • • September 1988 permit for septic system destruction. The permit <br /> indicates that three septic tanks were destroyed (4825). <br /> These permits are included in Appendix 4 of this report. <br /> 7.0 GOVERNMENT AGENCY RESEARCH <br /> 7.1 EPA Databases <br /> The California Environmental Protection Agency (EPA) manages contaminated <br /> sites through two sub-agencies: the State Water Resources Control Board <br /> (SWRCB) and the Department of Toxic Substances Control (DTSC). The <br /> SWRCB's GeoTracker database includes underground storage tank (UST) sites, <br /> leaking underground storage tank (LUST) sites, cleanup program sites, and land <br /> disposal sites. The database can be searched by reviewing two lists, the <br /> Permitted Underground Storage Tank (UST) Sites list and the Cleanup Sites list. <br /> DTSC's EnviroStor database includes Federal Superfund sites, State response <br /> sites, voluntary cleanup sites, school cleanup sites, corrective action sites, and <br /> permitted hazardous waste facilities. <br /> • <br /> LOGE 1712 4 <br />
