6.0 SITE DOCUMENTS <br /> 6.1 Environmental Health Department Records <br /> Hazardous Materials Records <br /> The San Joaquin County Environmental Health Department was contacted <br /> regarding the Site addresses of 12151 and 12385 South Jack Tone Road. No <br /> records related to storage tanks, hazardous materials, cleanups, or complaints <br /> were identified, according to Cheryl Field (personal communication, June 12, <br /> 2015). <br /> Well & Septic Permits <br /> Live Oak reviewed the San Joaquin County Environmental Health Department <br /> well and septic permit files for the Site addresses; eight permits were identified: <br /> • January 1976 permit for pump replacement at domestic well (12385). <br /> • February 1977 permit for pump replacement at irrigation well. <br /> • October 1978 permit for pump replacement at irrigation well (12385; <br /> appears to be off-site). <br /> • August 1981 permit for pump replacement at domestic well (12385). <br /> • October 1981 permit for new domestic well to the north of old well <br /> (12385). <br /> • November 2003 permit for new irrigation well (12385). <br /> • April 2005 permit for new residential septic system. The system utilized <br /> one 1,600-gallon septic tank, three 60' leach lines, and three 42"-diameter <br /> by 25'-deep seepage pits (12151). <br /> • November 2009 permit for addition of one 50' leach line and two 36"- <br /> diameter by 25'-deep seepage pits to existing residential septic system <br /> (12385). <br /> Copies of these permits have been included in Appendix 5 of this report. <br /> 7.0 GOVERNMENT AGENCY RESEARCH <br /> 7.1 EPA Databases <br /> The California Environmental Protection Agency (EPA) manages contaminated <br /> sites through two sub-agencies: the State Water Resources Control Board <br /> (SWRCB) and the Department of Toxic Substances Control (DTSC). The <br /> SWRCB's GeoTracker database includes underground storage tank (UST) sites, <br /> leaking underground storage tank (LUST) sites, cleanup program sites, and land <br /> disposal sites. The database can be searched by reviewing two lists, the <br /> Permitted Underground Storage Tank (UST) Sites list and the Cleanup Sites list. <br /> LOGE 1512 5 <br />
