6.0 SITE DOCUMENTS
6.1 Environmental Health Department Records
The EHD was contacted regarding the Site address of 18930 East Kettleman Lane,
Lodi. According to Cheryl Field of the EHD, no records are on file (personal
communication, October 30, 2012).
Live Oak requested copies of the EHD well and septic permit files for the Site
addresses. Three permits were identified:
• July 2006 permit for residential septic system. The permit indicates a 1,600-
gallon septic tank, two 40-foot leach lines, and two 36-inch diameter by 25-foot
deep seepage pits.
• September 2006 permit for new domestic well.
• July 2010 permit for new irrigation well. Based on the attached map, this well
appears to be on the adjacent parcel to the west.
7.0 GOVERNMENT AGENCY RESEARCH
7.1 EPA Databases
The California Environmental Protection Agency (EPA) manages contaminated sites
through two sub-agencies: the State Water Resources Control Board (SWRCB) and the
Department of Toxic Substances Control (DTSC). The SWRCB's GeoTracker database
includes underground storage tank (UST) sites, leaking underground storage tank
(LUST) sites, cleanup program sites, and land disposal sites. The database can be
searched by reviewing two lists, the Permitted Underground Storage Tank (UST) Sites
list and the Cleanup Sites list. DTSC's EnviroStor database includes Federal Superfund
sites, State response sites, voluntary cleanup sites, school cleanup sites, corrective
action sites, and permitted hazardous waste facilities.
Live Oak searched both the GeoTracker and Envirostor databases on October 29, 2012
for any listed sites within one-quarter mile of the Site. The subject Site was not listed on
either database, and no listed facilities were identified within one-quarter mile. `''>•Z- 3.5
7.2 Environmental Health Department Databases
Live Oak reviewed the EHD's Underground Storage Tank Site Mitigation List, dated July C . o
21 , 2011 , for surrounding facilities of concern. The subject Site was not listed, and no
listed facilities were identified within one-quarter mile of the subject Site. /3,23 `6
LOGE 1220 Page 4
