5.0 INTERVIEWS <br /> 5.1 Interview with Client <br /> Leslie Hiemstra, Ms. Van Groningen's daughter, was interviewed during the site <br /> visit on June 9, 2015. She indicated that she lives in the house but that a group <br /> of seven family members own the Site. She stated that they have owned the Site <br /> since approximately 2003, and the house was built in 2006. The domestic well <br /> and septic system were installed just prior to the construction of the house, she <br /> stated. <br /> Ms. Hiemstra indicated that, prior to its use as an orchard, the Site was planted <br /> with alfalfa and other crops. <br /> Ms. Hiemstra was not aware of any fuel tanks, chemical storage, automotive or <br /> equipment maintenance, dumping, or other environmental issues affecting the <br /> Site now or in the past. <br /> 6.0 SITE DOCUMENTS <br /> 6.1 Environmental Health Department Records <br /> Hazardous Materials Records <br /> The San Joaquin County Environmental Health Department was contacted <br /> regarding the Site addresses of 14141 and 13905 East Lone Tree Road. <br /> According to Cheryl Field, only one record was on file, a 2014 incident report <br /> regarding theft of fuel from a backup generator at the communications tower that <br /> resulted in a release of approximately two gallons of diesel to the ground <br /> (personal communication, June 12, 2015). The Environmental Health <br /> Department approved the cleanup and disposal of the affected soil. A copy of <br /> the incident report has been included in Appendix 4 of this report. <br /> Well & Septic Permits <br /> Live Oak reviewed the San Joaquin County Environmental Health Department <br /> well and septic permit files for the Site addresses; four permits were identified, all <br /> for the address of 14141 East Lone Tree Road: <br /> • November 2001 permit for cross-connect repair to irrigation well. <br /> • December 2005 permit for new domestic well at proposed home. <br /> y February 2006 permit for pump replacement at domestic well. <br /> LOGE 1510 4 <br />
