k4 KLEINFELDER <br /> Additionally, PHS/EHD will bill Home Depot USA Inc. for any oversight fees incurred above <br /> the initial 3 hour minimum and permit feet to be submitted by Kleinfelder. These fees will be <br /> billed directly to Home Depot USA Inc. by the PHS/EHD. Kleinfelder will forward the <br /> appropriate "billing and compliance acknowledgement forms" to Home Depot for signature. <br /> Signed originals of these forms must be submitted to PHS/EHD as part of the permit process. <br /> Kleinfelder will also contact Underground Service Alert 48 hours prior to conducting field work <br /> to notify local utilities of our proposed subsurface assessment. <br /> Task 2 -- Soil and Groundwater Samule Collection <br /> Kleinfelder proposes to collect soil and groundwater samples from three borings advanced using <br /> a truck mounted drill rig equipped with 8 inch diameter hollow stem auger. Soil samples will be <br /> collected at a depth of 5 feet and at a depth of 8 feet (anticipated to be above groundwater). The <br /> soil samples collected will be field screened using an organic vapor detector as described in <br /> Attachment B. Section B-2.2. The borings will be drilled, logged, and sampled following the <br /> protocol described in Attachment B, Section B-2.3. <br /> Each of the soil borings will be completed as a temporary monitoring well. The temporary wells <br /> will be used to obtain groundwater samples and depth to groundwater information. The use of <br /> temporary wells was selected over other one-time groundwater sampling methods (such as the <br /> Hydropunch or BAT-Probe sampling devices) due to Home Depot USA Inc.'s desire for <br /> relatively accurate depth to groundwater information. <br /> Three monitoring wells will be installed at the approximate locations shown on Plate 2. <br /> Groundwater is currently estimated to be at a depth of 7 to 10 feet in the area, but groundwater <br /> depth may be variable. Temporary two-inch wells will be constructed in the borings, as <br /> described in Attachment B, Section B-3.1. The wells will be screened from 5 feet above to 10 <br /> feet below first encountered groundwater. A sand pack will be placed around the well screen. <br /> Because the wells are intended to be destroyed immediately after sampling, the temporary wells <br /> will not be completed by installing a bentonite plug and grouting the annular space. <br /> The temporary wells will not be developed, but will be purged and sampled as described in <br /> Attachment B, Section B-4.2. Water level measurements will be obtained as described in <br /> Attachment B, Section B-4.1. The temporary wells will not be surveyed. Therefore, all <br /> groundwater measurements will be made relative to the existing ground surface. Prior to <br /> sampling and obtaining water level measurements, the wells will be allowed to equilibrate for a <br /> period of%2 to 1 hour. <br /> Upon completion of the soil and groundwater sampling the soil borings/temporary wells will be <br /> grouted the same day in accordance with PHS/EHD requirements. <br /> 20-YP9-022/20191'156 Page A-2 - <br /> Copyright 1999 Kleinfelder, Inc.
