In correspondence dated March 25, 2004 , SJC/EHD detailed the <br /> items that needed to be addressed and included in this work plan . The submitted work plan failed <br /> to include the following : <br /> 1 . A proposal for the installation of deep wells across the site to monitor the lateral extent of <br /> the contaminant plume at depth (as documented in deep monitoring well MW-4). <br /> 2 . A site-specific Health & Safety plan including a map with the route to the nearest hospital <br /> emergency room clearly marked . <br /> 3. An explanation about the discrepancies between the reported as-built depths of monitoring <br /> wells MW-1 , MW-3 and MW-4 and the actual depths measured at routine quarterly <br /> groundwater monitoring and sampling events . <br /> the form of an addendum as soon as possible, but no later than May 7, 2004 <br /> Submit these items in h f <br /> so that agency review of the work plan can continue. As you have been repeatedly out of <br /> compliance with regulatory deadlines , failure to submit the addendum by this date will result in <br /> referral of this site to the Regional Water Quality Control Board , Central Valley Region, or the San <br /> Joaquin County District Attorney's Office for enforcement action . <br /> Please note, as you have been previously directed in SJC/EHD correspondences dated <br /> April 11 , 2000, September 22, 2003 and November 14, 2003, routine quarterly groundwater <br /> monitoring and sampling of all site monitoring wells must continue until directed otherwise by this <br /> agency.
