Several medical room employees (one of which was working in the medical <br /> room of building D413) inquired regarding training pertaining to the hazardous waste container labeling and appeared <br /> unaware of the hazardous waste labeling requirements, furthermore, hazardous waste storage and labeling training <br /> was being provided by Mr. Sasson to the facility employees during the site walk through on September 4 and 9, 2014. <br /> The generator must ensure that all employees who handle hazardous waste are thoroughly familiar with proper waste <br /> handling and emergency procedures. Immediately provide training to all employees who handle hazardous waste and <br /> submit proof of training to the EHD. This is a minor violation. <br /> 108 CCR 66262.23(a)(4) Failed to send generator manifest copies to DTSC within 30 days. <br /> Generator copies of manifest number 006521995 FLE was not sent to DTSC within 30 days of shipment of hazardous <br /> waste. The generator manifest copy shall be submitted to DTSC for every shipment on a manifest when California is <br /> either the generator state or the destination or consignment state. The generator manifest copy shall be mailed to: <br /> y` DTSC Generator Manifests <br /> P.O. Box 400 <br /> Sacramento, CA 95812-0400 <br /> C Immediately mail a copy of the manifests listed and ensure that copies of all generator manifests are sent to DTSC <br /> within 30 days of shipping hazardous waste. This is a Class II violation. <br /> 303 CCR 66262.34(d)(2) Failed to maintain all communication or alarm systems, spill control, or decontamination <br /> equipment. <br /> Seven eyewash/shower stations were not properly maintained. These eye wash/shower stations were observed at the <br /> following locations with the following issues: in front of"plant ops" (not inspected/ no inspection label), in front of wood <br /> shop (not inspected/no inspection label), inside electrical room (not inspected/ no inspection label), mechanic shop <br /> (blocked access), laboratory (not tested due to lack of a floor drain, furthermore, Mr. Sasson stated that they know it's a <br /> problem and are actively working on correcting it), emergency room (actively leaking), in building C213 (needs additional <br /> liquid) . All facility communications or alarm systems, fire protection equipment, spill control equipment, and <br /> decontamination equipment, where required, shall be tested and maintained as necessary to assure its proper operation <br /> in time of emergency. Immediately begin this practice. This is a minor violation. <br /> 601 CCR 66262.34(d) Stored hazardous waste onsite greater than 180 days. <br /> Four 8-gallon black RCRA containers, observed in the main hazardous waste storage area, lacked an accumulation <br /> start date. Facilities who generate less than 1000 kg of hazardous waste per month and do not exceed 6000 kg of <br /> waste stored on site at any time may store waste on site up to 180 days. Immediately contact a licensed hazardous <br /> waste hauler to dispose of this waste under manifest and submit a copy of the manifest to the EHD.
