(ContiGroup) 9504 South Harlan Road French Camp, CA 95231 Supply Well Destruction Workplan Dear Mr. Wong: We are submitting the Wastewater Discharge Application, subsequent approved WDR (order No. R5-2004-0160), and the monitoring well installation permit fee ($89.00) for the above named facility. This information provides details of the approved activities for your review prior to procuring the permit for the installation of two additional monitoring wells. MONITORING WELL INFORMATION Huff & Huff, Inc., on behalf of ContiGroup Companies had previously submitted the document titled Proposed Pilot Study/Interim Action Zero-Valent Iron Treatment Cell (June 2004), which outlined the proposed scope of work. The scope of work includes soil fracturing/injection of emulsified zero-valent iron(EZVI) in the 60-80 foot depth range at three locations and the subsequent installation of two monitoring wells just beyond the radii of fracture/injection influence. The proposed monitoring wells are to be 4" diameter, installed to a depth of 80' bgs, and screened from 60'-80' bgs. Immediately following the submittal of the above-mentioned document, Huff & Huff, Inc. submitted the Wastewater Discharge Application (June 2004). We have included a copy of this document as an attachment for your records. The Central Valley Regional Water Quality Control Board approved the application on October 29, 2004 (Order No. R5-2004-0160). We have included a copy of this approval letter for your records.
