Neil O.Anderson&Associates HASP Page 2 of 21 <br /> ! March 18, 2014 <br /> ! 2.0 SITE DESCRIPTION <br /> The subject property is Neil O. Anderson & Associate's (NOA) Lodi Office. The site consists of a <br /> ! warehouse style building with offices, a large shop that consists of a laboratory, a fog curing <br /> concrete room (Fog Room), a maintenance shop, and miscellaneous equipment. The eastern <br /> section of the Site is a paved parking area and the southern portion is equipment, trailer and <br /> vehicle storage. <br /> ! The Fog Room is where concrete is cured by a Fog Curing Concrete System, which involves the <br /> frequent application of a fine fog mist to keep concrete samples moist. After the fog moistens <br /> ! the concrete, it drains into two (2) exterior drywells. The drywells are located just east of the <br /> center of the warehouse/office building. The resultant concrete curing waste water may <br /> contain high pH values and heavy metals. <br /> ! 3.0 ROLES AND RESPONSIBILITIES <br /> ! Personnel anticipated to be onsite at various times during site activities include: <br /> • Project Manager <br /> ! Senior Geologist <br /> • Field Team Leader <br /> • Field Personnel <br /> ! 3.1 Responsibilities of the Field Team Leader <br /> The Field Team Leader will direct the site activities. The Field Team Leader has the primary <br /> ! responsibility for ensuring that all personnel are aware of: <br /> • Names of personnel and alternates responsible for site safety and health. <br /> ! Safety, health, and other hazards present onsite. <br /> • Use of personal protective equipment (PPE) and ensuring that the equipment is <br /> available. <br /> ! Work practices by which the employee can reduce risks from hazards. <br /> • Safe use of engineering controls and equipment onsite. <br /> • Medical surveillance requirements, including recognition of symptoms which might <br /> ! indicate overexposure to hazards. <br /> • Site control measures, decontamination procedures, site standard operating procedures, <br /> and the contingency plan and responses to emergencies including the necessary PPE. <br /> ! The Field Team Leader is also responsible for ensuring that all employees have received at least <br /> 40 hours of health and safety instruction (offsite), and three days of actual field experience <br /> under the direct supervision of a trained experienced supervisor.
