The following containers did not have complete hazardous waste (HW) labeling: <br />a. 55 gallon drum of HW paint related - material / waste next to the paint gun cleaner does not have an <br />accumulation start date. <br />b. Three 55 gallon drums of HW solid - chromium and lead in the shop behind the office do not have <br />accumulation start dates. <br />c. 55 gallon drum of HW sludge and liquid in the shop behind the office does not have an accumulation <br />start date. <br />d. 55 gallon drum of used oil in the shop behind the office does not have an accumulation start date. <br />e. 55 gallon drum of used oil labeled HW solid - chromium and lead in the shop behind the office does <br />not have an accumulation start date. <br />f. 55 gallon white poly container partially full is labeled "gas & water" only. <br />g. 55 gallon drum of oily water west of the shop area was not labeled. <br />All hazardous waste containers shall be marked with the following information: <br />1. the words "Hazardous Waste" <br />2. name and address of generator <br />3. hazardous properties <br />4. physical state <br />5. composition (contents) <br />6. accumulation start date <br />Immediately label these containers and ensure that all containers are marked with all the required <br />information. Since it cannot be determined how long the containers list above have been on site, <br />immediately contact a licensed hazardous waste hauler to dispose of this waste under manifest and <br />submit a copy of the manifest to the EHD by June 11, 2011. <br />Notice to Comply <br />52. An emergency coordinator and modified contingency plan information is lacking. There must be at <br />least one emergency coordinator on site or on call to coordinate emergency response measures, and <br />the following information must be posted by a phone: the name and phone number of the emergency <br />coordinator; location of fire extinguishers, spill control equipment, and if present, fire alarm; and the <br />phone number of the fire department, unless the facility has a direct alarm. Immediately appoint an <br />emergency coordinator and post the required information by a phone. A form is provided that can be <br />used for this purpose. Submit proof of correction to the EHD by June 11, 2011. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE ($122). <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT END'S CURRENT HOURLY RATE. <br />EHD Inspe r <br />Received By: <br />Title: <br />- , w ` .
