MANTECA May 15, 2017 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II, or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 115 CCR 2715(a) Failed to inform the EHD of designated operator identification or change within 30 days. <br /> A new designated operator, Karen Arnaiz, was hired on January 1, 2017 and notification was not provided to the <br /> EHD within 30 days of the change. Any changes shall be reported to the EHD within 30 days. Immediately log into <br /> the California Environmental Reporting System (CERS) at http-.//cers.calepa.ca.gov and upload the notification <br /> identifying all the designated operators for this facility. <br /> This is a minor violation. <br /> 204 CCR 2712(i) Current monitoring plan approved by the EHD not found on site. <br /> An approved copy of the monitoring plans were not available for inspection. A copy of these plans shall be <br /> accessible on site at all times. Immediately locate and retain a copy on site. <br /> This is a minor violation. <br /> 209 CCR 2715(c) Designated operator did not inspect all required items and/or the inspection reports not completed. <br /> The designated operator (DO) failed to check and note the correct test dates for the secondary containment testing <br /> on the January through April 2017 designated operator monthly inspection reports. The DO inadvertently entered <br /> 5/26/16 instead of 5/26/15 for the secondary containment testing date. The DO inadvertently entered her ICC <br /> operator's expiration date as 5/5/15 instead of 5/5/17 on the DO monthly inspection reports for January through <br /> March 2017. During the monthly inspection, the designated operator shall check that all required testing and <br /> maintenance for the UST system have been completed and document the dates they were done. Ensure that <br /> designated operators performing monthly inspections at this facility are including all of the required information on <br /> the reports. <br /> This is a minor violation. <br /> 309 CCR 2636(f)(1) Monitoring in sump / UDC failed to activate an audible and visual alarm or stop the flow product. <br /> The Bravo box float and chain for the 87-product in under dispenser containment 3-4 failed to stop the flow of <br /> product at the dispenser when tested. All monitoring equipment shall be maintained to activate an audible and <br /> visual alarm or stop the flow of product at the dispenser when it detects a leak.
