The following manifest numbers were found without a signed off copy from the <br />destination facility. <br />008135371JJK (shipped 03/24/11), 008135372 JJK (03/24/11), 008815780JJK (09/15/11), <br />007119696JJK (02/26/10), 007431997JJK (09/26/10) <br />Hazardous waste generators shall retain copies of all manifests signed off by the destination facility on <br />site for three years and have them readily available for review. If the generator did not receive a copy of <br />the manifest with the handwritten signature of the owner or operator of the facility to which the <br />generator's waste was submitted within 35 days of the date the waste was accepted by the initial <br />transporter, the generator shall contact the transporter and/or the owner or operator of the designated <br />facility to determine the status of the waste. If the generator did not receive a copy of the signed off <br />copy of the manifest within 45 days of the date that the waste was accepted by the initial transporter, the <br />generator shall submit an Exception Report that includes a legible copy of the missing manifest, and a <br />cover letter signed by the generator explaining the efforts to locate the hazardous waste and the results <br />of those efforts. Exception reports shall be submitted to: <br />DTSC Report Repository, Generator Information Services Section <br />P.O. Box 806, Sacramento, CA 95812-0806 <br />Immediately locate a copy of the missing manifest or prepare and submit an exception report to DTSC. <br />Submit to the EHD a copy of the signed off copy of the manifests or a copy of the exception report <br />submitted to DTSC. <br />Submit to SJC EHD within 30 days (03/05/12): <br />1. one copy of the completed "Return to Compliance Certification" form <br />2. a written corrective action statement for each of the above violations <br />3. manifest/ disposal receipt for the waste oily -water and waste absorbent <br />Notes: <br />- Provided free Hazardous Waste labeling instructions <br />- In 2011 year, facility generated approximately 5900 gallons (23.6 Tons) of used oil and <br />approximately .5 tons of manifested hazardous waste = 24.1 Tons of hazardous waste <br />- Professional Engineering of hazardous waste oil tank assessment is currently in progress. Contractors <br />on site to ensure tanks are earthquake safe.
