(continued) <br /> - name &job title of each employee <br /> - description for each job title listed, including requisite skill, education, qualifications, and duties <br /> - description of training to be given to each position, both initial and as an annual refresher <br /> - records that training was completed for each employee <br /> Immediately update the hazardous waste management training for this facility and ensure that all <br /> employees complete this training within six months of hire and at least annually thereafter. Submit a <br /> copy of the updated training records to the EHD by April 6, 2014. REPEAT VIOLATION <br /> 121. Manifest number 009561888JJK (1/24/12) was found on site at the facility but was not on record <br /> with the Department of Toxic Substances Control (DTSC). According to Ms. Hall, the facility does not <br /> mail any of the manifests to the state, and they instead rely on their transporter to take care of all mailing <br /> and notification requirements. Within 30 days of each shipment, a generator shall submit a legible copy <br /> of each manifest used to DTSC. Immediately submit a copy of this manifest to DTSC and ensure that <br /> copies of all future manifests are submitted to DTSC within the required time frame. <br /> 122, 123, & 124. Manifest numbers 008135372JJK (3/24/11) and 011357287JJK (4/22/13) were found <br /> without a signed off copy from the destination facility. Hazardous waste generators shall retain copies of <br /> all manifests signed off by the destination facility on site for three years and have them readily available <br /> for review. If the generator did not receive a copy of the manifest with the handwritten signature of the <br /> owner or operator of the facility to which the generator's waste was submitted within 35 days of the date <br /> the waste was accepted by the initial transporter, the generator shall contact the transporter and/or the <br /> owner or operator of the designated facility to determine the status of the waste. If the generator did not <br /> receive a copy of the signed off copy of the manifest within 45 days of the date that the waste was <br /> accepted by the initial transporter, the generator shall submit an Exception Report that includes a legible <br /> copy of the missing manifest, and a cover letter signed by the generator explaining the efforts to locate <br /> the hazardous waste and the results of those efforts. Exception reports shall be submitted to: <br /> DTSC Report Repository <br /> Generator Information Services Section <br /> P.O. Box 806 <br /> Sacramento, CA 95812-0806 <br /> Immediately locate a copy of the missing manifests or prepare and submit an exception report to DTSC. <br /> By April 6, 2014, submit to the EHD a copy of the signed off copy of the manifests or a copy of the <br /> exception report submitted to DTSC.
