Submit a copy of the updated training records to the EHD by April 6, 2014. REPEAT VIOLATION 121. Manifest number 009561888JJK (1/24/12) was found on site at the facility but was not on record with the Department of Toxic Substances Control (DTSC). According to Ms. Hall, the facility does not mail any of the manifests to the state, and they instead rely on their transporter to take care of all mailing and notification requirements. Within 30 days of each shipment, a generator shall submit a legible copy of each manifest used to DTSC. Immediately submit a copy of this manifest to DTSC and ensure that copies of all future manifests are submitted to DTSC within the required time frame. 122, 123, & 124. Manifest numbers 008135372JJK (3/24/11) and 011357287JJK (4/22/13) were found without a signed off copy from the destination facility. Hazardous waste generators shall retain copies of all manifests signed off by the destination facility on site for three years and have them readily available for review. If the generator did not receive a copy of the manifest with the handwritten signature of the owner or operator of the facility to which the generator's waste was submitted within 35 days of the date the waste was accepted by the initial transporter, the generator shall contact the transporter and/or the owner or operator of the designated facility to determine the status of the waste. If the generator did not receive a copy of the signed off copy of the manifest within 45 days of the date that the waste was accepted by the initial transporter, the generator shall submit an Exception Report that includes a legible copy of the missing manifest, and a cover letter signed by the generator explaining the efforts to locate the hazardous waste and the results of those efforts. Exception reports shall be submitted to: DTSC Report Repository Generator Information Services Section P.O. Box 806 Sacramento, CA 95812-0806 Immediately locate a copy of the missing manifests or prepare and submit an exception report to DTSC. By April 6, 2014, submit to the EHD a copy of the signed off copy of the manifests or a copy of the exception report submitted to DTSC. REPEAT VIOLATION ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
