The following ian itemized list of hazardous waste violations that have not been <br /> addresse I <br /> for Global Materials Management, LLC as of April 17, 2017. <br /> Open violations from August 26, 2016 inspection <br /> Violation #114- Failed to keep signed copy of manifests from the designated facility for three years. <br /> Per Mr. Kandathil the busir ess has been operating at this location for"a couple, 3-4 months". At time of inspection <br /> only one hazardous waste disposal manifests was available for review. Mr. Kandathil stated others were at his office <br /> in Southern California. <br /> A generator shall keep a c py of each signed manifest for at least three years from the date the waste was <br /> accepted by the initial tran porter. <br /> Within 30 days, provide the EHD with copies of all hazardous waste manifests from this location. Begin the practice <br /> of maintaining hazardous aste manifests for a minimum of three years. <br /> Violation #117 - Failed to omplete training on hazardous waste management and emergency response <br /> procedures. <br /> According to Mr. Kandathil, employees have not received hazardous waste training. Mr. Kandathil is waiting to train <br /> employees until he feels th y will "work out". Personnel who handle hazardous waste shall be trained within six <br /> months of assignment and ake part in an annual review of the initial training received. At a minimum, the training <br /> program shall be designed o ensure that facility personnel are able to respond effectively to emergencies by <br /> familiarizing them with eme gency procedures, emergency equipment, and emergency systems, including where <br /> applicable: <br /> 1. procedures for using, inspecting, repairing, and replacing facility emergency and monitoring equipment; <br /> 2. key parameters for a tomatic waste feed cut-off systems; <br /> 3. communications or a rm systems; <br /> 4. response to fires or a plosions; <br /> 5. response to ground- ater contamination incidents; and <br /> 6. shutdown of operatio s. <br /> The owner or operator shall maintain the following documents and records at the facility: <br /> 1. the job title for each sition at the facility related to hazardous waste management, and the name of each <br /> employee filling each job; <br /> 2. written job descriptior for each position listed above; <br /> 3. written description of he type and amount of both introductory and continuing training that will be given to <br /> each person filing positions isted above; <br /> 4. records that docurner t that the training has been given to and completed by facility personnel. <br /> Training records on current mployees shall be kept until closure of the facility. Training records on former <br /> employees shall be kept for t least three years from the date the employee last worked at the facility. Immediately <br /> provide employees with initi I or refresher hazardous waste training. Submit a copy of the roster and the syllabus to <br /> the EHD. <br /> Violation #119 - Failed to eep copies of consolidated manifesting receipts for three years. <br /> Per Mr. Kandathil the busin ss has been operating at this location for"a couple, 3-4 months". At time of inspection <br /> copies of consolidated mani ests were not available for review. <br /> A generator shall keep a co y of each consolidated manifest for at least three years. <br /> Within 30 days, provide the HD with copies of all consolidated manifests from this location.
