C. Maintain equipment in good condition, with all safeguards in place when in <br />operation; <br />d. Report all work related injuries and illnesses immediately to the area <br />supervisor; and <br />e. Report all unsafe acts and, equipment and conditions immediately to the <br />area supervisor. <br />6.0 MANAGEMENT OF HAZARDOUS SUBSTANCES <br />6.1 Procurement of Approved Hazardous Substances <br />6.1.1 When requesting procurement of a chemical material, the requestor must verify <br />that the material is approved for receipt by the QBR Stockton facility AND is <br />listed on the facility's current Approved Chemical Materials List. <br />6.1.2 When requesting the procurement of an approved chemical material, the <br />originating requestor shall provide to the facility's Purchasing Department, for <br />each chemical material, the following: <br />A. The facility assigned MSDS Number; <br />B. Hazardous Substance designation, if applicable; <br />C. Any special management requirements referenced on the ACM List, if <br />applicable. <br />6.2 Procurement of New Hazardous Substances <br />6.2.1 When requesting the procurement of a new or non -approved chemical <br />material, it shall be the responsibility of the originating requestor to <br />provide the facility's Environmental Engineer (EE) with a current MSDS for <br />the subject chemical material. <br />NO QBR EMPLOYEE SHALL PROCURE, BRING ON-SITE AND/OR <br />RECEIVE ANY CHEMICAL MATERIAL WHICH HAS NOT BEEN <br />APPROVED BY THE FACILITY'S EE PRIOR TO RECEIPT IN <br />ACCORDANCE WITH THIS HSMP. <br />6.2.2 It is the responsibility of the QBR Stockton Facility's EE to maintain and <br />keep current the facility's Approved Chemical Materials (ACM) List in <br />accordance with this Plan. <br />6.3 Purchasing Department Responsibilities <br />6.3.1 Upon receipt of a request to procure a chemical material, the facility's <br />Draft HMMP <br />March, 2000 HMMP-5 <br />
