information is required, return the packet to the requestor within three (3) <br />working days of receipt with an explanation of deficient and/or additional <br />information, including confirmation and/or update of the status of any <br />MSDS with an issue date older than one (1) year. <br />2. Review the completed chemical material review packet, and, if applicable, <br />classify the chemical material as either or both of the following: <br />a. Hazardous Substance, as defined by this Plan; and <br />b. Extremely or Acutely Hazardous Material or Substance (AHM), as <br />defined in 40 CFR 355, Appendix A. No AHM will be approved for <br />receipt by the QBR facility until all of the following has been <br />accomplished: <br />The requirements of QBR Facility's HWMP and HSMP have <br />been met; and <br />ii. An Risk Management Program has been developed in <br />accordance with Article 2, Chapter 6.95 of the California <br />Health and Safety Code; <br />3. If the chemical material is classified as a hazardous substance, identify <br />and identify proper hazardous substance management procedure(s) (i.e., <br />handling, labeling and storage conditions) in accordance with this Plan. <br />4. Review the completed packet and determine the following: <br />a. Whether a waste will be generated by the use of the chemical <br />material as described by the requestor. <br />b. If a waste is generated, a description of the waste, including: <br />I. Physical state (liquid, solid or mixture); <br />ii. Hazardous or nonhazardous; <br />5. If hazardous, determine the hazardous constituent(s) and concentration <br />and amount(s) generated per application and on a quarterly basis <br />(anticipated). <br />6.7.5. If a hazardous waste is generated, It is the responsibility of the EE to determine <br />whether the waste is compatible with the facility's existing wastestreams, and if <br />so, provide the QBR assigned hazardous wastestream identification number. <br />6.7.6. If the waste is hazardous and incompatible with existing QBR hazardous <br />wastestreams, the facility's EE or designee is responsible for insuring that the <br />Draft HMMP <br />March, 2000 HMMP-12 <br />
