DRAFT <br />X Personnel making the evaluation and audit; <br />X Date(s) of the inspections; and <br />X Major observations relating to the implementation of the SWPPP. <br />1.8.3 Annual Report <br />By July 1 of each year, QBR=s SWPPT will submit an ANNUAL REPORT for <br />Storm Water Programs to the Central Valley Regional Water Quality Control <br />Board (see Table 1). The ANNUAL REPORT must include the following: <br />1. A summary of the facility's observation reports; <br />2. A summary of the Storm Water Monitoring Program's laboratory <br />results, if applicable; <br />3. The site's Annual CSCE Summary Report; and <br />4. A certification that the facility is in compliance with the conditions of <br />the GENERAL PERMIT. The Annual Report must be signed by the <br />owner and the owner must certify, under penalty of law, that the <br />document and all of its attachments were prepared under the direction <br />of the owner and that the information is accurate and complete. <br />1.8.4 SMP Revisions <br />If QBR=s CSCE Report lists changes at the facility that may have a significant <br />effect on the potential for the discharge of pollutants to surface waters, the <br />site=s SWPPT shall amend the QBR=s SWPPP accordingly. These changes <br />may include construction activity, or changes in operations or maintenance <br />activities. Modifications to the SWPPP which improve the effectiveness of the <br />site's storm water management efforts will also be described and <br />documented. <br />1.8.5 Reporting Requirements <br />In addition to the Annual Reporting requirements described above, QBR=s <br />SWPPT will initiate the following notifications, when applicable: <br />X Planned Facility Changes: The SWPPT representative will notify <br />the State Water Resources Control Board (SWRCB) of any of the <br />C:\TEMP\SMPREV2.doc Section 1-31 <br />
