1 <br /> 1 <br /> ' 2.0 MONITORING WELL INSTALLATION REPORT <br /> The objective of the monitoring well installation was further assessment of the lateral and <br /> ' vertical extent of petroleum-hydrocarbon impact beneath the Site. The site-specific objective <br /> was to assess groundwater quality within the `B" and "C" water-bearing zones. PSC installed <br /> one groundwater monitoring well, CT-MW-8-87 in the "B" water-bearing zone. PSC <br /> ' attempted to install two groundwater monitoring wells in the "C" water-bearing zone beneath <br /> the Site. However, soil sampling and drill cuttings from a soil boring drilled on the west side <br /> of the Site, revealed no permeable or saturated soil from 87 feet to 130 feet bgs. Therefore, a <br /> well was not installed in this location. <br /> The monitoring wells were constructed and installed in accordance with California <br /> ' Department of Water Resources guidelines. The three borings and two groundwater- <br /> monitoring wells were identified using the CT abbreviation for the Site, MW, well number, <br /> and final depth (e.g. CT-MW-8-87). Well locations are shown on Figure 3. A California <br /> Professional Geologist provided the technical guidance for the construction and installation of <br /> the monitoring wells. <br /> 2.1 Pre-Field Procedures <br /> Upon approval of the well installation work plan by CVRWQCB, PSC obtained the required <br /> SJCEHD boring permits to perform the groundwater monitoring well installation activities. <br /> The boring permit applications were completed, submitted, and approved. PSC coordinated <br /> the field schedule with the property owner to obtain site access to perform the installations. <br /> PSC obtained and coordinated schedules with coring and drilling subcontractors to perform <br /> the various related tasks. <br /> PSC prepared a Health and Safety Plan (HASP) for PSC personnel and their subcontractors <br /> ' that performed on-site field activities. All personnel were informed of the health and safety <br /> concerns regarding the work to be performed and the Site conditions. Daily safety meetings <br /> were held prior to starting field activities. <br /> PSC marked the general borehole location areas on the property and indicated total distances <br /> for utility clearance in white paint in the street or on the sidewalk in front of the Site for <br /> Underground Service Alert (USA). PSC notified USA and scheduled the utility locate upon <br /> approval of the work plan by CVRWQCB and upon receipt of an approved boring permit <br /> from SJCEHD. PSC scheduled Subdynamic Utility Detection Services, Inc. (SubDynamic) to <br /> perform the third-party utility locating and borehole location clearance, as well. <br /> 2.2 Soil Boring and Well Installation Procedures <br /> ' Each borehole was cleared of subsurface utilities using a hand auger, jack hammer, or air <br /> knife. Material was removed, to a depth of approximately 5 feet bgs using a vacuum truck. <br /> The soil debris and water generated during the borehole clearance activities and drilling was <br /> Lstored in DOT-approved 55-gallon drums and subsequently transported to a licensed waste <br /> disposal facility for proper disposal.
