Manifest numbers 005923360JJK (12/23/09), 005547514JJK (2/19/09), 000516430JJK (2/18/09), 000516429JJK (2/18/09), and 004090036JJK (4/15/08) were not found on site. Hazardous waste generators shall retain copies of all manifests for three years or until a copy signed off by the destination facility is received. The signed off copy shall be retained and made readily available for review for three years. If the generator did not receive a copy of the manifest with the handwritten signature of the owner or operator of the facility to which the generator's waste was submitted within 35 days of the date the waste was accepted by the initial transporter, the generator shall contact the transporter and/or the owner or operator of the designated facility to determine the status of the waste. If the generator did not receive a copy of the signed off copy of the manifest within 45 days of the date that the waste was accepted by the initial transporter, the generator shall submit an Exception Report that includes a legible copy of the missing manifest, and a cover letter signed by the generator explaining the efforts to locate the hazardous waste and the results of those efforts. Exception reports shall be submitted to: DTSC Report Repository Generator Information Services Section P.O. Box 806 Sacramento, CA 95812-0806 Immediately locate a copy of the missing manifests or prepare and submit an exception report to DTSC. By March 12, 2011, submit to the EHD a copy of the signed off copy of the manifests or a copy of the exception report submitted to DTSC. REPEAT VIOLATION 64. Copies of spent lead acid battery disposal records were not found on site. The generator shall retain on site a legible copy of each manifest or bill of lading which identifies spent lead acid batteries shipped to a person who stores, uses, reuses, recycles, or reclaims the batteries or their components. Immediately locate a copy of all disposal records for spent lead acid batteries for the last three years, maintain them on site, and submit copies to the EHD by March 12, 2011. REPEAT VIOLATION 85. Copies of universal waste disposal records for alkaline batteries and electronic devices were not found on site. A generator of universal waste shall keep a record of each shipment of universal waste on site for at least three years. The record may take the form of a log, invoice, manifest, bill of lading, or other shipping document, and shall include the following: ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE ($122).
