The contingency plan for this facility did not include descriptions of emergency response to fire or explosion, a list of emergen y equipment at the facility, the phone number of Cal EMA, or the name, address, and phone numbe of the facility Emergency Coordinator. A contingency plan shall include all of the following: -actions facility personnel shall take in response to fires, explosions, and unplanned release to air, soil, or surface water -arrangements agreed to by local police departments, fire departments, hospitals, contractors, and state & local emergency response teams -names, addresses & phone numbers (office & home) of the Emergency Coordinator -list of all emergency equipment at the facility (fire, spill, communications/alarms, decon), including location, physical description, and a brief outline of its capabilities -evacuation plan, including routes and signals to be used -telephone number of Cal EIOA This information must be kept current at all times. Immediately update the contingency plan for this facility to include all of the necessary information and submit a copy of the updated plan to the EHD by August 8, 2011. 49. The training records for this facility only included a record that training was conducted for each employee. Facility personnel shall complete a training program in accordance with Title 22 regulations. This program shall be directed by a person trained in hazardous waste management procedures and shall include the following: - hazardous waste management procedures, including contingency plan implementation - emergency procedures, equipment, and systems - key parameters for automatic waste feed cut-off systems - communications or alarm systems - response to fire, explosion,; or groundwater contamination incident - shut down of operations The training program shall b completed by each employee within six months of hire, prior to unsupervised work, and sha I be reviewed annually. Training records shall be maintained on site until closure of the facility and shall include: - name &job title of each employee - written job description for each position (job title) listed, including requisite skill, education, qualifications, and duties - description of training to be given to each position - records that training was completed for each employee ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122).
