The contingency plan for this facility did not include descriptions of emergency response to fire or <br /> explosion, a list of emergen y equipment at the facility, the phone number of Cal EMA, or the name, <br /> address, and phone numbe of the facility Emergency Coordinator. A contingency plan shall include all <br /> of the following: <br /> -actions facility personnel shall take in response to fires, explosions, and unplanned release to air, soil, <br /> or surface water <br /> -arrangements agreed to by local police departments, fire departments, hospitals, contractors, and state <br /> & local emergency response teams <br /> -names, addresses & phone numbers (office & home) of the Emergency Coordinator <br /> -list of all emergency equipment at the facility (fire, spill, communications/alarms, decon), including <br /> location, physical description, and a brief outline of its capabilities <br /> -evacuation plan, including routes and signals to be used <br /> -telephone number of Cal EIOA <br /> This information must be kept current at all times. Immediately update the contingency plan for this <br /> facility to include all of the necessary information and submit a copy of the updated plan to the EHD by <br /> August 8, 2011. <br /> 49. The training records for this facility only included a record that training was conducted for each <br /> employee. Facility personnel shall complete a training program in accordance with Title 22 regulations. <br /> This program shall be directed by a person trained in hazardous waste management procedures and <br /> shall include the following: <br /> - hazardous waste management procedures, including contingency plan implementation <br /> - emergency procedures, equipment, and systems <br /> - key parameters for automatic waste feed cut-off systems <br /> - communications or alarm systems <br /> - response to fire, explosion,; or groundwater contamination incident <br /> - shut down of operations <br /> The training program shall b completed by each employee within six months of hire, prior to <br /> unsupervised work, and sha I be reviewed annually. Training records shall be maintained on site until <br /> closure of the facility and shall include: <br /> - name &job title of each employee <br /> - written job description for each position (job title) listed, including requisite skill, education, <br /> qualifications, and duties <br /> - description of training to be given to each position <br /> - records that training was completed for each employee <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122). <br /> THIS FACILITY IS UBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector: Re By: Title: <br /> EHD 23-02-003 ev 0 0/10 CONTINUATION FORM <br />
