The training record for the facility lack employee job title and job description. Facility personnel shall complete a training program that teaches them to perform their duties in a way that ensures the facility's compliance with Title 22 regulations. This program shall be directed by a person trained in hazardous waste management procedures and shall include the following: hazardous waste management procedures relevant to the positions in which they are employed, including contingency plan implementation, emergency procedures, equipment, and systems, key parameters for automatic waste feed cut-off systems, communications or alarm systems, response to fire, explosion, or groundwater contamination incident, shut down of operations The training program shall be completed by each employee within six months of hire, prior to unsupervised work, and shall be reviewed annually. Training records shall be maintained on site until closure of the facility and shall include: name &job title of each employee, written job description for each position (job title) listed, including requisite skill, education, qualifications, and duties, description of training to be given to each position, records that training was completed for each employee Immediately include the job title and job description to the facility's employee training records and submit the EHD within 30 days of receiving this report. 60, 61, & 62. Manifest numbers -002708798FLE, 002708858FLE were not found on site. Hazardous waste generators shall retain copies of all manifests for three years or until a copy signed off by the destination facility is received. The signed off copy shall be retained and made readily available for review for three years. If the generator did not receive a copy of the manifest with the handwritten signature of the owner or operator of the facility to which the generator's waste was submitted within 35 days of the date the waste was accepted by the initial transporter, the generator shall contact the transporter and/or the owner or operator of the designated facility to determine the status of the waste. If the generator did not receive a copy of the signed off copy of the manifest within 45 days of the date that the waste was accepted by the initial transporter, the generator shall submit an Exception Report that includes a legible copy of the missing manifest, and a cover letter signed by the generator explaining the efforts to locate the hazardous waste and the results of those efforts. Exception reports shall be submitted to: DTSC Report Repository Generator Information Services Section P.O. Box 806 Sacramento, CA 95812-0806 Immediately locate a copy of the missing manifests or prepare and submit an exception report to DTSC. Within 30 days of receiving this report, submit to the EHD a copy of the signed off copy of the manifests or a copy of the exception report submitted to DTSC. ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
