The training record for the facility lack employee job title and job description. Facility personnel shall <br /> complete a training program that teaches them to perform their duties in a way that ensures the facility's <br /> compliance with Title 22 regulations. This program shall be directed by a person trained in hazardous <br /> waste management procedures and shall include the following: hazardous waste management <br /> procedures relevant to the positions in which they are employed, including contingency plan <br /> implementation, emergency procedures, equipment, and systems, key parameters for automatic waste <br /> feed cut-off systems, communications or alarm systems, response to fire, explosion, or groundwater <br /> contamination incident, shut down of operations <br /> The training program shall be completed by each employee within six months of hire, prior to <br /> unsupervised work, and shall be reviewed annually. Training records shall be maintained on site until <br /> closure of the facility and shall include: name &job title of each employee, written job description for <br /> each position (job title) listed, including requisite skill, education, qualifications, and duties, description of <br /> training to be given to each position, records that training was completed for each employee <br /> Immediately include the job title and job description to the facility's employee training records and submit <br /> the EHD within 30 days of receiving this report. <br /> 60, 61, & 62. Manifest numbers -002708798FLE, 002708858FLE were not found on site. Hazardous <br /> waste generators shall retain copies of all manifests for three years or until a copy signed off by the <br /> destination facility is received. The signed off copy shall be retained and made readily available for <br /> review for three years. If the generator did not receive a copy of the manifest with the handwritten <br /> signature of the owner or operator of the facility to which the generator's waste was submitted within 35 <br /> days of the date the waste was accepted by the initial transporter, the generator shall contact the <br /> transporter and/or the owner or operator of the designated facility to determine the status of the waste. <br /> If the generator did not receive a copy of the signed off copy of the manifest within 45 days of the date <br /> that the waste was accepted by the initial transporter, the generator shall submit an Exception Report <br /> that includes a legible copy of the missing manifest, and a cover letter signed by the generator <br /> explaining the efforts to locate the hazardous waste and the results of those efforts. Exception reports <br /> shall be submitted to: <br /> DTSC Report Repository <br /> Generator Information Services Section <br /> P.O. Box 806 <br /> Sacramento, CA 95812-0806 <br /> Immediately locate a copy of the missing manifests or prepare and submit an exception report to <br /> DTSC.
