Contact numbers are provided in <br /> Appendix E of this SPCC Plan; <br /> o Record the information shared during the notification call on <br /> the Discharge Notification Form in Appendix D and attach a <br /> copy to this SPCC Plan; and <br /> o Coordinate cleanup and obtain assistance from one or more <br /> cleanup contractor(s) or other response organization(s) as <br /> necessary. <br /> If the Designated Person Accountable for Discharge Prevention is not <br /> available at the time of the discharge, then designated emergency <br /> coordinator assumes responsibility for coordinating response activities. <br /> 5.3.2 Information and Procedures for Spill Reporting <br /> A summary sheet is included in Appendix D to facilitate reporting. Northwest Pipe <br /> Company also has an internal Spill Reporting Form, located on the Northwest Pipe <br /> Company Intranet site. The person reporting the discharge must provide the <br /> following information: <br /> • Name, location, organization, and telephone number <br /> • Name and address of the party responsible for the incident (if any) <br /> • Date and time of the incident <br /> • Location of the incident <br /> • Source and cause of the release or discharge <br /> • Type of material(s) released or discharged <br /> • Quantity of material(s) released or discharged <br /> • Danger or threat posed by the release or discharge <br /> • Number and types of injuries (if any) <br /> • Media affected or threated by the discharge (i.e., water, land, air, etc.) <br /> • Weather conditions at the incident location <br /> • Any other information which may help emergency personnel respond to the <br /> incident <br /> Contact information for reporting a discharge to the appropriate authorities is listed <br /> in Appendix E. <br /> 5.3.3 Discharge Report to USEPA Regional Administrator <br /> Under certain circumstances, 40 CFR § 112.4(a) requires information be submitted <br /> to the United State Environmental Protection Agency (USEPA) Regional <br /> Administrator and the appropriate state agency in charge of oil pollution control <br /> activities (see contact information in Appendix E) whenever the facility discharges <br /> (as defined in 40 CFR § 112.2) more than 1,000 gallons of oil in a single event, or <br /> discharges (as defined in 40 CFR § 112.2) more than 42 gallons of oil in each of two <br /> discharge incidents within a 12-month period. <br />
