VIOLATIONS: <br />CLASS 1 VIOLATIONS <br />None. <br />CLASS_2 VIOLATIONS <br />1. MANIFEST ERRORS. <br />On November 114, 1996, Physics International had violated Title <br />22, California Code of Regulations (Cal. Code Regs.), section <br />66262.20(a) in that five manifests contained errors, as follows: <br />a. Manife$t 95144156 did not have the description of the <br />hazardous waste in Box J - the only description was a <br />generi� DOT description (Non-RCRA Hazardous Waste, Solid) <br />and a profile number. <br />b. Manifest 95847850 did not have the facility's BOE <br />identification number in Box B. <br />C. Manifests 95144152, 95144153, and 95144154 contained <br />corrections that were not initialed by the operator. <br />2. OUT-OF-DATE CONTINGENCY PLAN. <br />On November 14ections Physics6 261 34 (a) (4) International <br />66265.54 (d) Title <br />in that <br />Cal. Code Regs., s <br />the primary emergency coordinator was not corrected even though the <br />personnel change had taken place about four months previously. <br />3. OPEN CONTAINERS. <br />On November 14, 1996, Physics International violated Title tin <br />Cal. Code Regs., sections 66261.34 (a) (1) (A) and 66265.173(a) <br />that two 55 -gallon steel drums had open bungs with funnels in the <br />bungs. These were located um lation areas at Waste was not beingnadded <br />and near the vehicle maintenance shop <br />or removed at the time. <br />
