SAN JOAQUIN COUNTY <br /> ENVIRONMENTAL HEALTH DEPARTMENT <br /> 600 East Main Street, Stockton, California 95202-3029 <br /> Telephone:(209) 468-3420 Fax:(209)468-3433 Web:www.sogov.org/ehd/unitiii.html <br /> CONTINUATION FORM Page: 5 of 7 <br /> OFFICIAL INSPECTION REPORT Date: 11/16/11 <br /> Facility Address: 1010 Shaw, Stockton Program: HW <br /> SUMMARY OF VIOLATIONS <br /> CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> (Violation #108, continues from page 4) <br /> Generator shall make hazardous waste determination of the waste and manage the waste accordingly. <br /> * Immediately make a hazardous waste determination of the above wastes and manage each <br /> accordingly, or dispose of the wastes as hazardous waste in accordance to California Code of <br /> Regulations (CCR) Title 22. Immediately stop recycling metal fines and make a hazardous and size <br /> determination: Particles 100 microns or smaller must be handled as hazardous waste if the metal is <br /> determined to be a hazardous waste. If a waste is declared non-hazardous, please submit documents <br /> supporting such a determination to EHD. Submit a statement and supporting documentation (i.e. <br /> hazardous waste disposal receipts) explaining how the above wastes were managed by December 19, <br /> 2011. <br /> 122, 123, 124 125. Copies of hazardous waste disposal records were not available for inspection on site <br /> and (known) Manifest number for 2009: #001747659SKS, 001942050SKS, 002151714SKS, and <br /> 006064117JJK, for 2010: #002184340SKS, 2286334SKS, 003329555FLE, and 003731042FLE, and for <br /> 2011: #002596486SKS were not found on site. Hazardous waste generators shall retain copies of all <br /> uniform manifests signed off by the disposal facility and all receipts used in a consolidated manifesting <br /> procedure on site for three years and have them readily available for review. If the generator did not <br /> receive a copy of the manifest with the handwritten signature of the owner or operator of the facility to <br /> which the generator's waste was submitted within 60 days of the date the waste was accepted by the <br /> initial transporter, the generator shall submit a legible copy of the missing manifest, with some indication <br /> that the generator has not received confirmation of delivery. This information shall be submitted to: <br /> DTSC Report Repository <br /> Generator Information Services Section <br /> P.O. Box 806 <br /> Sacramento, CA 95812-0806 <br /> * Immediately locate a copy of the missing manifest (including any not listed above) or prepare and <br /> submit the required information to DTSC, and a copy of all consolidated hazardous waste manifests and <br /> receipts for the last three years, maintain them on site, and submit copies to the EHD by December 19, <br /> 2011. <br /> 128.
