The analytical report and chain-of-custody documentation are presented in <br /> Attachment 4 <br /> Monitoring Well Installation <br /> All four monitoring wells (MW-2, MW-3, MW-4 and MW-5)were constructed using 2 <br /> inch OD Schedule 40 PVC casing with 10 feet of 0.010 inch slotted PVC well screen. <br /> A 2/12 sand filter pack was placed around each well screen from the bottom of the <br /> borehole to approximately two feet above the top of the screen interval. <br /> Approximately two feet of TR30 bentonite chips were placed on top of the filter pack. <br /> The remainder of the borehole annulus was grouted with tremie piping to <br /> approximately one foot bgs.A traffic rated 12-inch diameter flush mounted well box <br /> was installed at each well location. Well construction details are summarized in <br /> Table 2. Detailed boring logs with well construction details are included as <br /> Attachment 5. <br /> Groundwater Monitoring Well Development and Sampling <br /> The existing and newly installed groundwater monitoring wells were developed by <br /> Cascade. and sampled by ARCADIS between June 26 and June 27, 2008. <br /> Groundwater samples were collected in accordance with California Environmental <br /> Protection Agency(CaIEPA) procedures outlined in Representative Sampling of <br /> Groundwater for Hazardous Substances(CaIEPA, 1995). Gettler-Ryan Incorporated <br /> (G-R)will conduct quarterly monitoring and sampling of monitoring wells MW-1 <br /> through MW-5 beginning with the third quarter of 2008. <br /> Field Procedures <br /> An approximate ten-well-volume purging and sampling technique of the previously <br /> existing monitoring well (MW-1)and the four newly installed monitoring wells (MW-2, <br /> MW-3, MW-4 and MW-5)was performed with a decontaminated stainless steel bailer <br /> for groundwater purging and sample collection.
