For off-site facilities, the EPA identification number or name <br /> and address (if un-numbered) of each hazardous waste producer from which <br /> waste was recieved. <br /> d. Description and quantity of each hazardous waste stored, treated, <br /> or received during the previous year (arranged by EPA identification number <br /> of producer for off-site facilities) . <br /> e. Method of treatment, storage, or disposal for each waste. <br /> f. Certification signed by owner, operator, or representative. <br /> Sharpe submits the report to the Department of Health Services on or <br /> before March 1 for the preceding year. Sharpe' s 1987 Annual Report is shown <br /> in Annex H and includes the information required, in the detailed list above. <br /> The report must include, to extent feasible, a description of the <br /> efforts undertaken during the year to reduce the volume and toxicity of waste <br /> generated. Also, a description of the changes in volume and toxicity of <br /> • waste actually achieved during the year in comparison to previous years. <br /> B. Availability and Retention of Records <br /> The following two items describe the accessibility of all records and <br /> reports of the hazardous waste storage facilities and the procedures in place <br /> to ensure the retention of the documents. <br /> 1. Required records are available for inspection by DHS, State Water <br /> Resources Control Board, or the Regional Water quality Control Board. <br /> As stated previously in Section XII.A.l.b, the records and reports <br /> are submitted to the Environmental Coordinator by the wastewater treatment <br /> plant operator and available to any regulatory agency member upon request. <br /> 2. Records are retained for periods identified. <br /> Once records have been received from the wastewater treatment plant <br /> operator, the Environmental Coordinator will continue to file these records <br /> until the facility closes. <br /> XII-4 <br /> (10/23/90) <br /> • <br />
