tSupplemental Investigation Report,Problem Assessment Report and Work Plan Cleanup and Abatement Order No S-OD-707„ _„ E4I Protect Na 1232 • The samples will be kept chilled until delivered to the laboratory for analytical testing COC record forms will be used to document sample collection and shipment to the laboratory for analytical testing The COC record form identifies the contents of each shipment, the analytical testing to be completed on each sample, and maintains the custodial integrity of the samples 5.6 Field Log Book A bound field log book with consecutively numbered pages will be maintained ntained to document and record where, when, how and from whom any vital project information was obtained Other written records such as COC record forms and memoranda to file may be used in consort with the field log book All entries in the field log book will be made in indelible, water proof ink In the event of an error, a single solid line will be made through the error, the new notation made and the initials of the person making the change marked next to the change Entries in the field log book will include • Site name and location • Recorder's name • Team members • Time of Site arrival and departure • Other personnel on-site, if any o Work complete that event or day o Deviations from sampling plan Information recorded during the collection of each sample will include • Sample identification number ® Date and time of sample collection • Type of sample, e g , grab or composite • EAI project number a Name of client • Name of sampler Preservatives added, if any • Type of sampling equipment used • Decontamination procedures used e Field observations and details important to analysis and/or integrity of the samples • Sketch showing sampling locations, identification numbers, distances, benchmarks, etc SS 1232P IRW WP -17- ENVIRONMENTAL 1!- ENVIRONMENTAL AUDIT, IAIC.®
