Ms. Stacy Rivera <br />San Joaquin County EHD <br />#S Violation text: The contingency plan for this facility did not include <br />descriptions of emergency response to fire or explosion, a list of emergency <br />equipment at the facility, the phone number for the Cal EMA, or the name, <br />address, and phone number of the facility Emergency Coordinator. A <br />contingency plan shall include all of the following: <br />- actions facility personnel shall take in response to fires, explosions, and <br />unplanned release to air, soil, or surface water <br />- arrangements agreed to by local police departments, fire departments, <br />hospitals, contractors, and state and local emergency response teams <br />- names, addresses, and phone numbers (office & home) of the Emergency <br />Coordinator <br />- list of all emergency equipment at the facility (fire, spill, <br />communications/alarms, decon), including location, physical description, <br />and a brief outline of its capabilities <br />- evacuation plan, including routes and signals to be used <br />- telephone number of Cal EMA <br />This information must be kept current at all times. Immediately update the <br />contingency plan for this facility to include all of the necessary information and <br />submit a copy of the updated plan to the EHD. <br />The contingency plan has been updated to include the necessary information. A <br />copy of updated contingency plan is included as part of this response. <br />#6 Violation text. A written training program and training records were not <br />available on site. Facility personnel shall complete a training program in <br />accordance with Title 22 regulations. This program shall be directed by a <br />person trained in hazardous waste management procedures and shall include <br />the following: <br />- hazardous waste management procedures, including contingency plan <br />implementation <br />- emergency procedures, equipment, and systems <br />- key parameters for automatic waste feed cut-off systems <br />- communications or alarm systems <br />- response to fire, explosion, or groundwater contamination incident <br />- shut down of operations <br />The training program shall be completed by each employee within six months <br />of hire, prior to unsupervised work, and shall be reviewed annually. Training <br />records shall be maintained on site until closure of the facility and shall <br />include: <br />- name and job title of each employee <br />- written job description for each position (job title) listed, including requisite <br />skill, education, qualification, and duties <br />- description of training to be given to each position <br />- records that training was completed for each employee <br />
