Job title for each position at the facility related to hazardous waste44r:4'TU nCD5QT4nFMT <br />management, and the name of each employee filling each job. <br />2. Written job description for each employee listed above <br />3. Written description of the type and amount of both introductory and <br />continuing training that will be given to each person filling positions <br />above; <br />4. Records that document that the training has been given to and <br />completed by facility personnel. <br />Training records on current employees shall be kept until closure of the facility. <br />Training records on former employees shall be kept for at least three years form <br />the date the employee last worked at the facility. Immediately provide <br />employees with initial or refresher hazardous waste training. Submit a copy of <br />the roster and the syllabus to the EHD. <br />Ryder requires yearly training on hazardous waste management procedures for all <br />employees. New employees are to be trained immediately upon hiring. Included <br />as part of this response is the roster of employees trained at the January 20, 2015 <br />training and a copy of the yearly training components and information specific to <br />hazardous waste and emergency spill response training (Attachment A). <br />#2 Violation #111— Failed to maintain complete personnel training records — <br />employees training records for the management of hazardous waste were <br />incomplete. The records did not include a list of each employee with their job <br />titles. (The job titles on site in the training records were Technician, Service <br />Attendant, Customer Service Coordinator/Service Writer.) Employee training <br />records shall include: <br />1. Job title for each position at the facility related to hazardous waste <br />management, and the name of each employee filling each job. <br />2.
