REPEAT SEP 15 2015 <br />VIOLATION �-NVIRONMENTAL <br />Yl=agTu nC0A4TkAC41T <br />A contingency plan has been developed to include the following information: <br />- Actions facility personnel shall take in response to fires, explosions, and <br />unplanned release to air, soil, or surface water <br />- Arrangements agreed to by local police departments, fire departments, <br />hospitals, cleanup contractors, and state and local emergency response teams <br />- Names, addresses, and phone number (office and home) of the Emergency <br />Coordinator and any alternatives <br />- List of all emergency equipment at the facility (fire, spill, <br />communications/alarms, decontamination), including location, physical <br />description, and a brief outline of its capabilities <br />- Evacuation plan, including at least two routes and signals to be used <br />- Telephone number of Cal EMA <br />A copy of this plan is included as part of this response (Attachment Q. <br />#2 Violation text: #135 — HSC 25185(c)(3) — Failed to submit a written response <br />within 30 days of receiving an inspection report. The Return to Compliance <br />paperwork for the August 19, 2013 inspection was not submitted. A deficiency <br />letter was issued on December 2, 2013. The operator of the site or facility <br />which receives an inspection report shall submit a written response to the <br />department or the local officer or agency within 60 days of receipt of the <br />inspection report, or within a shorter time as the department or local agency <br />requires. This response shall include a statement documenting the corrective <br />actions that have been or will be taken by the operator. Immediately submit all <br />necessary follow up paperwork for the 2013 hazardous waste generator <br />inspection to the EHD. REPEAT VIOLATION <br />The Return to Compliance paperwork from the August 2013 inspection is <br />included as part of this response (Attachment G). <br />Open violations from the August 19, 2013 inspection: <br />#1 Violation text: #110 — CCR 66265.52 — contingency plan incomplete. The <br />contingency plan for this facility did not include the descriptions and <br />capabilities of the emergency equipment at the facility, including the cartridge <br />respirators, chemical protective suites, chemical protective gloves, fire <br />extinguishing system, and absorbent.
