Quik Stop g Markets August 14, 1990 <br />Project No. 349132-41 Page 2 <br />The Fremont Office Safety Coordinator and the Project Manager are responsible for <br />reviewing and authorizing the provisions of this Plan (as designated by their signatures <br />on each page of this Plan). <br />The Project Supervisor is responsible for the submittal of this plan to the Project <br />Foreman and for advising the Project Foreman on health and safety matters. He has the <br />authority to provide for the auditing of compliance with the provisions of this plan, <br />suspend or modify work practices, and to administer disciplinary actions for individuals <br />whose conduct does not meet the requirements set forth herein. The Project Supervisor <br />will ensure that the following items are adequately addressed: <br />• Safety Supplies and Equipment Inventory <br />• Medical Surveillance Program/Physical Examinations <br />• Training Programs/Hazard Communication <br />• Accident/Incident Reporting Procedures <br />• Decontamination/Contamination Reduction Procedures <br />The Project Foreman and the Project Geologist are responsible for the dissemination of <br />the information contained in this plan to site personnel and to the responsible <br />representative of each subcontracting firm working under Exceltech, Inc. on the <br />project. <br />The Project Foreman will act as the Site Safety Officer. During the geological <br />investigation when the Project Foreman is not present, the Project Geologist will act as <br />the Site Safety Officer. The Site Safety Officer is responsible for ensuring that the <br />following items are adequately addressed: <br />• Ensure that all personnel on-site adhere to the requirements of the project <br />safety plan. <br />• Obtain acknowledgement, on signoff page enclosed as Attachment A, from all <br />on-site personnel or visitors regarding their acceptance of the project safety <br />plan requirements. <br />• Investigate all incidents and accidents and report in writing to the Project <br />Supervisor who will report to the Corporate Safety Officer. <br />• Ensure that proper personal decontamination points are available. <br />The Project Supervisor and the Site Safety Officer have the authority to suspend work <br />anytime they determine that the provisions of the plan are inadequate to ensure worker <br />safety. The Site Safety Officer shall also inform the Project Supervisor of individuals <br />whose conduct is not consistent with the requirements of the plan. <br />
