Pershing, Stockton San Joaquin County Public Health Services, Environmental Health Division (PHS- EHD) , has received your November 14, 1989 groundwater monitoring report. We share your concern about the .increasing contamination levels being recorded from monitoring well #2. A proposal for further assessment and/or remediation for the above referenced site was submitted to this office September 27, 1989. California Regional Water Quality Control Board's (CVRWQCB) comments on this report are attached. PHS-EHD has the following recommendations: a) Slug tests utilizing "known weight" cylinders are a preferred method instead of introducing liquid into the monitoring wells. b) Encroachment permits, if applicable, should be submitted to this office along with the application for monitoring well permit and $35.00 permit fee. All proposals and/or reports submitted must be accompanied by a Letter of. Acknowledgement from the responsible party(ies) which states, at a minimum, the following: I(We) declare, under penalty of perjury, that the information and or recommendations contained in the attached proposal or report is/are true and correct, and that all work and reports which required geologic or engineering evaluations and/or judgments have been performed under the direction of an appropriately registered or certified professional. The attached proposal or report utilizes the "Regional Board Staff Recommendations For Initial Evaluation and Investigation of Underground Tanks" and "Appendix All reporting format.
