WHAT IS INCLUDED IN PLAN <br /> All HMMP's consists of four components. If a location has underground storage tanks, <br /> then two additional components are required, <br /> I_The first component is a business activities form. This form includes the locations <br /> facility identification, the types of hazardous material activity performed, documents if <br /> the location has underground or above ground storage tanks on site,and identifies if the <br /> location stores extremely hazardous materials or has any other local HMMP <br /> requirements. <br /> 11. The second component is the Business owner/operator identification form. This form <br /> includes the facility identification number,address, business owner, environmental <br /> contact,on-site emergency contact personnel, any required additional local information, <br /> the mailing/billing information, and the name of the individual certifying the information <br /> in the plan.. This form must be kept current at all times, <br /> 111.The third component is an inventory of all hazardous materials stored on site. The <br /> HMMP must include the following 55 gallon drummed products stored on a hazardous <br /> materials inventory chemical description form at a minimum: 15W40 motor oil, EP-2 <br /> moly-grease, automatic transmission fluid, 75-90 weight synthetic tube, 50 WT synthetic <br /> tube)and 200cf cylinders (oxygen, acetylene,argon). If a location stores used oil,diesel <br /> fuel or gasoline in any container 55 gallons or larger, these must also be included. Some <br /> counties may require other products, but these are the minimum standard, <br /> IV.The fourth component of every location's HMMP is a site map. The site map must <br /> identify the location of where each of the following are on site: all hazardous material <br /> listed on a chemical description form, buildings on site,emergency response equipment, <br /> first aid equipment, drainage lines, underground and above ground storage tanks, and <br /> utilities, <br /> UNDERGROUND STORAGE TANK LOCATION REQUIREMENTS <br /> Monitoring and Emergency Response Plans <br /> If a location has underground storage tanks,then the 14-MMP must also include an <br /> Underground Storage Tank Monitoring Plan and an Underground Storage Tank <br /> Emergency Response Plan. These plans identify how the locations underground storage <br /> tanks are monitored and what to do in the event of an unauthorized release of product <br /> from the tank(s), <br /> A locations underground storage tank monitoring plan requires the storage tank electronic <br /> monitoring system,associated tank piping, sumps, spill buckets, secondary containment <br /> system piping and sumps to have tests performed to ensure the integrity of the equiprrient. <br /> Each test must provide passing test results. If failures of any of the tests performed <br /> occur, the system must be investigated for the cause of the test, repairs made and the <br /> system retested. <br />
