The PM will ensure that management performs an investigation of all <br /> incidents or accidents which had the potential to cause a lost-time or hospitalization incident <br /> or fatality within 24 hours of the incident. <br /> The PM for this job is Sydney Geels. <br /> 2.3 Project Health and Safety Manager <br /> The Project HS Manager is responsible for the preparation and modification (as necessary) of <br /> this HASP. The Project HS Manager will approve changes and update the HASP as <br /> warranted by altered site conditions and shall have the only authorization to effect such <br /> changes (except those changes outlined in the Emergency Response Plan). The Project HS <br /> Manager will advise the PM on health and safety issues which may have an impact on project <br /> operations. In addition, responsibility of the Project HS Manager includes: <br /> General health and safe program administration; <br /> ' safety Pr gr <br /> • Provide technical assistance to the PM and the Project FC; <br /> • Investigate significant accidents, illnesses and near-misses. Recommend <br /> corrective actions as appropriate. Review all Accident/Incident Investigation <br /> Reports; <br /> • Determine required personal protective equipment for each work area; <br /> • Assist the FC in establishing decontamination area locations; <br /> • Evaluate and approve contractors regarding health and safety compliance both <br /> prior to accepting the contract and upon completion of the project, as <br /> appropriate; and <br /> • Conduct exposure monitoring to determine type and extent of personnel exposure <br /> and to assess the appropriateness of protective measures. <br />' The Project HS Manager is Mark Freiberg. Mr. Freiberg is a Certified Industrial Hygienist <br /> (CIH) and a Certified Safety Professional (CSP). <br /> 2.4 Project Field Coordinator <br />' The Project Field Coordinator (FC) reports to the PM and is responsible for field <br /> implementation of the HASP. This includes communicating project health and safety <br /> requirements to all on-site project personnel (both IT and subcontractor personnel), field <br /> supervision, consultation with the Project HS Manager regarding changes to the HASP and <br /> conducting monthly health and safety inspections.
